The form collects personal identifying information from applicants or incumbents seeking employment with the Government or working under contract, including name, date and place of birth, contact information, and U.S. citizenship status. Provision of information is voluntary, but false or inaccurate statements may result in felony charges under 18 U.S.C. § 1001, including fines up to $10,000 and/or imprisonment up to five years. All personally identifiable information is protected under the Privacy Act and used solely for determining suitability to serve in the designated position.
