a. Volume I – General. Offerors are required to submit a completed SF 1449 (including acknowledgment of Amendments). All final monetary extensions shall be in whole dollars only. Failure to follow the below Contract Proposal preparation instructions may cause your proposal to be deemed unacceptable by the Government. The General Volume shall be organized as follows and contain the identified information.
(1) TAB A, Exceptions/Assumptions (if required) – Identify and explain any exceptions, deviations, or assumptions used in the proposal. Prior to the closing date identified in the 1449, the Offeror shall contact the Contracting Officer (IAW paragraph A.2.a.(3)) in order to gain clarification and/or approval of any exceptions, deviations, or assumptions regarding this solicitation.
(2) TAB B, SF 1449 – The 1449 shall be fully completed upon submission. The offeror is cautioned that the SF 1449 must contain an original signature in block in block 30a of the form. The contractor shall acknowledge any amendments to the RFP in accordance with the instructions on the SF 1449 with Addendum to FAR 52.212-1, Instruction to Offerors. The Subcontracting Plan is not a requirement for evaluation in source selection, but rather, a requirement for award, and it will be incorporated into any resultant contract.
b. Volume II – Technical Capability.
(1) General. The Technical Capability Volume shall be clear, concise, and include sufficient detail for effective evaluation and for substantiating the validity of stated claims in the Offeror’s proposal.
Legibility, clarity and coherence are very important. Your responses will be evaluated against the Technical Capability defined in Addendum to Clause 52.212-2, Evaluation Factors for Award. The proposal should not simply rephrase or restate the Government’s requirements, but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Statements that the offeror understands, can, or will comply with the PWS (including referenced publications, technical data, etc.); statements paraphrasing the PWS or parts thereof (including applicable publications, technical data, etc.); and phrases such as “standard procedures will be employed” or “well known techniques will be used,” etc., will be considered unacceptable. Offerors shall assume that the Government has no prior knowledge of their facilities and experience, and will base its evaluation on the information presented in the offeror’s proposal. Elaborate brochures, documentation, binding, detailed art work, or other embellishments are unnecessary and are not desired.
(2) The Technical Capability Volume shall, at a minimum, be prepared in a form consistent with the Performance Work Statement (PWS) and the evaluation criteria for award set forth in Addendum to Clause 52.212-2 of this solicitation. This section shall be prepared in an orderly format and in sufficient detail to enable the Government to make a thorough evaluation of the contractor’s technical competence and ability to comply with the contract task requirements specified in the PWS. The offeror shall address as specifically as possible the actual methodology you would use for accomplishing the PWS tasks. The Technical Capability Volume shall be organized according to the following general outline:
(1) Table of Contents
(2) List of Table and Drawings (If any)
(3) Evaluation Criteria
(a) Factor 1: Technical Capability
(i) Sub-Factor A: Staffing Approach
(ii) Sub-Factor B: Quality Control Approach
(iii) Sub-Factor C: Management Approach
(a) Factor 1 - Technical Capability. The technical proposal shall emphasize the offeror’s technical capability in providing food services. The proposal shall discuss in sufficient detail the overall understanding of the requirement, the feasibility of the offeror’s staffing approach, and the expertise required to perform all required services. The technical proposal shall address the following subfactors, with specific information as described below:
(b) Sub-factor A - Staffing Approach:
(i) The Offeror shall demonstrate to the Government its staffing approach and labor mix to provide high quality Full Food Service IAW PWS paragraphs 2.1.4, 2.1.7, 2.1.11, and Technical Exhibit F. At a minimum, the offeror shall provide a labor matrix(ices) by facility. The offeror shall clearly show the productive man-hours for all necessary staffing by facility full time equivalents (FTEs) split between different labor categories. The matrix shall demonstrate adequacy of productive man-hours to accomplish the workload and effective supervision of personnel by facility. A justification for each proposed labor category shall be provided. All qualifications and cross utilization of the labor force shall be clearly identified, explained, and depicted. An explanation shall be provided about how the cross utilization of the labor force does not increase performance risk under the PWS. A justification for each proposed labor category shall be provided. If no cross utilization is being offered, the contractor shall state, “There are no cross utilization of positions offered”.
(ii) Explain how the key personnel positions identified in PWS 2.2.3 fit into the overall organization and describe the proposed responsibilities for each position. Identify personnel proposed for those positions and provide narrative and documentation that demonstrates the knowledge, capability, and/or qualifications required in PWS 2.2.3.
(c) Sub-factor B - Quality Control Approach: Provide an approach to develop a quality control program including methods to prevent, identify, and correct deficiencies in the quality of services.
Include a description of the inspection techniques and process control methods to adequately ensure performance as outlined in PWS sections 2.1.8, 3.1, 4.1 and 4.2.
(d) Sub-factor C - Management Approach:
(i) Provide a narrative that clearly outlines the management approach, techniques, or procedures to be utilized to accomplish the basic functions of planning, organizing, and controlling each food service program requirement as set forth in the PWS section 2.1.7 and the proposed interface between functional areas for project management and administration. Include a phase-in approach.
(ii) Provide a description of the management approach to be used to provide normal and extended services, particularly when accommodating workload fluctuations resulting from changes in operational requirements. The proposal must address the offeror’s ability to respond to frequent short notice/no notice operational changes IAW PWS Section 2.1.4 and menu planning IAW PWS Section 3.1.2.
(iii) Provide an approach as to how Government Furnished Property
(GFP) will be administered. This will include a description of the approach to 1) develop and disseminate operation instructions, procedures, and control directives in preparation for the assumption of responsibilities and accountability for all GFP, 2) conduct initial and ongoing inventories (PWS 3.2.1), and conduct preventive maintenance.
(iv) Provide a proposed cleaning schedule for each dining facility as stated in the PWS 3.0 and 3.1.
c. Volume III – Past Performance. The Past Performance evaluation will be accomplished by reviewing aspects of an offeror's recent and relevant Past Performance, focusing on and targeting performance which is relevant to the effort.
(1) Past Performance information described herein is required on the offeror and all subcontractors, teaming partners, and/or joint venture partners proposed to perform over 25% percent of the proposed effort based on the total proposed price. The offeror shall submit, along with the information required in this paragraph, a consent letter, executed by each subcontractor, teaming partner, and/or joint venture, authorizing release of adverse past performance information to the offeror so the offeror can respond to such information (Attachment 1). For each identified effort for a commercial customer, the offeror shall submit a client authorization letter, authorizing release to the Government of requested information on the offeror’s performance (Attachment 2).
(2) In conducting the Past Performance evaluation, offerors are cautioned that the
Government may use data provided in the offeror’s proposal and data obtained from other sources. These sources include, but are not limited to, the Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and integrity Information System (FAPIIS), interviews with Program Managers, Contracting Officers, Defense Contract Management Agency (DCMA) and commercial sources. Offerors are reminded that both independent data and data provided by offerors in their proposals may be used by the Government to evaluate offeror past performance. However, the burden of providing thorough, complete, and current past performance information as requested in this paragraph remains with the offerors. Proposals that do not contain the information requested by this paragraph, risk rejection or a less than an acceptable performance rating by the Government. All past performance comments received will be taken into account and could affect the overall rating. The overall past performance evaluation is a subjective decision based on the whole of all data received. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.”
(3) The offeror shall include documentation regarding their relevant past performance as it directly relates to the work being procured under this solicitation. The offeror SHALL NOT go back any further than 3 years for the submitted data. The past performance data shall document a successful history of past contract performance. To illustrate the offeror’s past performance, the offeror shall submit a Past Performance Volume containing the following:
(a) Table of Contents.
(b) Summary Page describing the role of the offeror and each subcontractor, teaming partner, and /or joint venture partner that the offeror is required to provide Past Performance Specific Relevant Contract Reference Sheets for the past performance experience in accordance with Attachment 3 to this solicitation.
(c) Consent Letters executed by each subcontractor, teaming partner, and/or joint venture partner, authorizing the release of past performance information so the offeror can respond to such information. A sample consent letter is attached as Attachment 1 to this solicitation.
(d) Client Authorization Letters for each identified effort for a commercial customer authorizing release to the Government of requested information on the offeror’s performance. A sample authorization letter is attached as Attachment 2 to this solicitation.
(e) Organization Structure Change History - Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc. In many cases, these changes have taken place during the time of performance of relevant present or past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult to determine what past performance is relevant to this acquisition. To facilitate this relevancy determination, include in this proposal volume a "roadmap" describing all such changes in the organization of your company. As part of this explanation, show how these changes impact the relevance of any efforts you identify for past performance evaluation/performance confidence assessment. Since the Government intends to consider present and past performance information provided by other sources as well as that provided by the offeror(s), your "roadmap" should be both specifically applicable to the efforts you identify and general enough to apply to efforts on which the Government receives information from other sources.
(f) Specific Relevant Contracts Format – Provide specific relevant contracts in accordance with Attachment 3. Information provided for each relevant contract and is limited to five (5) pages. A maximum of three (3) relevant contracts will be accepted per proposal. Officers are encouraged to submit at least one (1) relevant contract for each critical subcontractor, teaming partner, and/or joint venture partner to demonstrate relevant past performance in their proposed scope. The offeror shall provide documentation outlining the offeror’s past performance with contracts, as a prime or major subcontractor, which is the same or similar in nature, size, and complexity to the services being procured under this Solicitation. The submittal shall include rationale supporting your assertion of relevance and how it was determined that the work performed previously was the same or similar in nature, size, and complexity to the work specified by this solicitation. Offerors are required to explain what aspects of the contracts are deemed relevant to the proposed effort, and to what aspects of the proposed effort they relate.
d. Volume IV - Cost/Price: The Offeror shall complete Blocks 19-24 of the SF 1449. For the purpose of price evaluation, the Contractor shall include a Pricing Matrix, by contract period that details in its proposed monthly fixed prices and a narrative explaining its prices, vacation etc. The Volume shall be organized according to the following outline:
(1) Table of Contents
(2) Pricing Matrixes/spreadsheet
(3) Pricing Narrative
(1). The Offeror’s spreadsheet shall include all proposed labor (FTE/PTE), Occupation
Code and Title used, identify each position as exempt or nonexempt, basic labor rates, fully burden rates, proposed vacation hours for each FTE/PTE, and identify all labor that belongs to the subcontractor. The pricing tables shall not be locked and will be submitted in Microsoft Excel. The Government reserves the right to require the submission of Other Than Certified Cost or Pricing Data as is deemed necessary to arrive at a fair and reasonable price.
(2). Phase-In: The 14 day phase-in period shall be separately priced. NOTE: If the reporting of phase-in period is not separately priced, insert “NSP” in the blank shown.
(3). Contract Manpower Reporting CLINs: If the reporting of Manpower Equivalents is not separately priced, insert “NSP” in the blank shown.
(4). Options: Evaluation of options shall not obligate the Government to exercise the option(s). The offeror's total price for the purpose of evaluation will include the Phase in base period and all option periods. The Offeror shall not submit a separate price for the six month option to extend services under FAR Clause 52.217-8. The Government will add one-half of the offeror's final option period and add this price to the offeror's total price. As such, the offeror's total price will include the phase-in, base period, 1st option, 2nd option, 3rd option, 4th option and 1/2 of the 4th option.
(5) Offerors are not required to submit other than certified cost and price data with their proposals. However, offerors should be aware that it may be required later in the selection process in order to determine price reasonableness.

SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE RELEASE OF PAST
PERFORMANCE INFORMATION TO THE PRIME CONTRACTOR
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the Government's Past Performance Evaluators in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Past Performance Volume III Proposal.
