About this file
This document is a Statement of Work (SOW) for the decontamination of an indoor firing range and bullet recovery room at the U.S. Army Criminal Investigation Laboratory. The contractor will annually decontaminate all horizontal surfaces in these areas by wet-wiping, HEPA vacuuming, or a combination of both to remove lead dust. The contractor must have specific qualifications, such as lead abatement training and HAZWOPER certification. The work will be performed on-site and access to facilities is limited. The government will use a Quality Assurance Surveillance Plan (QASP) to monitor the contractor's performance in areas such as management responsiveness, contract administration, schedule control, and technical performance. The contractor will be rated as Exceptional, Acceptable, or Unacceptable, and the government may take action for unacceptable performance. The contractor will provide all labor and equipment/resources required to perform the decontamination work, which includes servicing and decontaminating the bullet recovery system and bullet trap, and properly disposing of all waste generated.

STATEMENT OF WORK
DECONTAMINATE INDOOR FIRING RANGE
C.1.0 General
C.1.1 Scope of Work
1. Decontaminate Indoor Firing Range: The Contractor will annually decontaminate all horizontal surfaces in the indoor firing range and bullet recovery room. Decontamination will consist of wet-wiping, High Efficiency Particulate Air (HEPA) vacuuming, or a combination of both to remove lead dust from these locations. Wipe samples will be taken to verify cleanliness after decontamination.
C.1.2 Background Information
Wipe sampling results from samples taken in December 2009 in the bullet recovery room and indoor firing range have shown a level of contamination that warrants action on the part of USACIL in order to limit employee and family member exposure to lead. The highest results found were 234ug/ft2 in the bullet recover room and 5,980ug/ft2 in the indoor firing range.
C.1.3 Personnel
Hazardous Waste Operations Technician, HAZMAT Responder or Lead Abatement Specialist
Qualifications:
• Has had Lead Abatement training within the past 12 months.
• Has had 40-hour HAZWOPER course or refresher within the past 12 months
• Work under the direction of a Certified Industrial Hygienist, Certified Hazardous Materials Manager, or
Certified Safety Professional
Place of Work
On-site at the US Army Criminal Investigation Laboratory, 4930 N. 31st Street, Building 925, Forest Park, GA 30297.
C.1.8 Access to Facilities
Access to laboratory will be limited to work location and arranged with facility manager.
C.1.11 Quality Assurance
1. Purpose of Contract: The purpose of this plan is to identify the methods and procedures the Government will use to insure it receives the services under contract as identified in the Statement of Work (SOW). This plan will focus on the level of performance required by the SOW, not the methodology or process.
2. Review Responsibilities: The government will designate the Contracting Officer’s Representative (COR) for this contract. This Quality Assurance Surveillance Plan (QASP) will be used for this contract since most of the labor is time and material. The COR will be responsible for interfacing with the contractor in the performance of this contract.

Page 2 of 5 Pages
3. Methods and Frequency:
(a) Surveillance will be accomplished on a continual and random basis. The method used for evaluation will be based on schedules, reports, observation, customer feedback, adherence to proposed costs, and review of deliverables.
(b) Timely feedback to the contractor on unacceptable performance will be provided by the COR to the Contracting Officer.
(c) A summary of any items that do not meet the terms and conditions of the contract shall be prepared by the COR and forwarded to the Contracting Officer (CO).
(d) A checklist and summary of performance shall be prepared by the COR, and submitted in support of unacceptable performance if identified during the performance period.
(e) The checklist provided at table A shall be used by the COR in monitoring this contract.
At a minimum, monitoring shall take place on a bi-weekly basis or more often, at the option of the COR and CO.
4. Performance Areas:
(a) Management Responsiveness. Contractor’s responsiveness will be evaluated on timeliness of deliverables, informing Government of work progress, and identification of problems and effective corrective action.
(b) Contract Management/Administration. Contractor management/administration will be evaluated on the quality and effectiveness of the contractor's management of the contract. The contractor shall use the hours authorized to provide support throughout the period of performance. The contractor will be evaluated in the successful control of resources. The COR shall:
• Ensure that hours logged are accurate to the best of his/her knowledge.
• Review monthly progress reports and ensure that work is on schedule.
• Ensure that all materials/equipment procured under this order are turned over to the
COR, and remain the property of the Government.
(c) Schedule Control. Contractor’s schedule will be evaluated on overall adherence to contract or task order schedules.
(d) Performance. The contractor will be evaluated as to the quality of output of their work. The contractor's personnel should be technically competent in the functional area(s) identified in the task order. Included in the technical performance are the contractor’s contributions in meetings and reviews, the quality of the contractor’s technical reports, contractor's productivity, and the overall quality of the technical support provided to the government.
5. Evaluation Criteria:
(a) Exceptional (E). E indicates performance clearly exceeds contractual requirements.
The area of evaluation contains few minor problems for which corrective actions appear highly effective.
(b) Acceptable (A). A indicates performance clearly meets contractual requirements.
The area of evaluation contains some minor problems for which the corrective actions appear satisfactory.
Page 3 of 5 Pages
(c) Unacceptable (U). U indicates the contractor is in danger of not being able to satisfy contractual requirements and recovery is not likely in a timely manner. The area of evaluation contains serious problems for which the corrective actions appear ineffective.
(d) Upward or downward arrows may be used to indicate an improving or worsening trend insufficient to change the assessment status.
6. Documentation: Documentation used and referenced to perform surveillance will consist of contractor plans and procedures, schedules, and customer feedback. The COR shall use Surveillance Logs to record information on scheduled observations and defects noted for time and material tasks. Each observation is recorded on the log. The document(s) then become a formal record for reference. The contractor is told each time a defect is found during scheduled observations. The COR makes a notation on the surveillance log of the date and time the defect was discovered, then, the COR asks a contractor task leader to initial the notation, documenting notification of the defect only.
7. Documenting Unacceptable Performance:
a. The COR attempts to resolve the problem with the contractor. The checklist and the attempted solution (along with an evaluation of results) are forwarded to the CO.
b. The contractor shall complete a response to the unacceptable evaluation if requested by the COR, CO or by its own choice.
8. Taking Action:
a. The COR may evaluate the contractor's performance and document any non-compliance, but only the CO may take action against the contractor for an unacceptable rating.
b. When the contractor's performance is unacceptable and a formal action is indicated and make a recommendation to the CO.
9. Documentation: During the course of the contract, the COR retains a copy of all inspections and checklists. At the end of the period, the COR shall forward these records to the CO for inclusion in the contract file. However, when a specific service becomes unsatisfactory during a surveillance period, the inspection documentation supporting the contract discrepancy report is forwarded to the KO.
10. Rating: All contractor tasks will be rated E, A, or U on the surveillance log. The COR will collect the checklists and necessary supporting documentation and will then give an overall site rating for each of the performance areas.
11. Surveillance Logs and Report: The checklists will be used to record observations and defects, and rate individual contractor tasks. The Surveillance Report will be used to give overall ratings in the performances areas.

5. The contractor will certify the cleanliness of the decontamination work by taking a minimum of
12 wipe samples on horizontal surfaces. The U.S. Department of Housing and Urban Development protocols for wipe sampling will be followed and a minimum of 3 samples will be taken in each of the bullet recovery room and indoor firing range before and after cleaning and decontamination.
