a. Offerors shall provide proof of DoD Advanced Geophysical Classification Accreditation Program
(DAGCAP) [submit under this Factor]. If the Offeror is in a joint venture with, or is in a teaming agreement with a
DAGCAP accredited Geophysical Classification Organization (GCO), the Offeror shall explain in detail how all work performed in relation to AGC will be organized and executed under the GCO’s quality system. The Offeror shall comply with sections 5.2 and 7.1.1 of the DOD Quality System Requirements where Quality Control

b. Indicate the workforce size, capabilities, and organizational structure of the team as a whole, to include subcontractors or teaming partners (if applicable). The Government should be able to easily see the depth, diversity of technical disciplines and years of experience of the Offeror’s qualified personnel.
c. Include organizational relationships with any proposed subcontractor or teaming partners, and describe to what extent the Prime has successfully worked with the proposed subcontractors and teaming partners in the past.
Include locations and functions of significant organizational offices.
d. Present an organizational chart that clearly defines the roles and responsibilities and lines of authority for all personnel and subcontractors proposed. The chart should clearly identify reporting lines of authority and communication.
e. Provide a list of subcontractors to be utilized under this contract, how it will be determined if they are included in a particular Task Order, and what their role would be. If an Offeror proposes to use a DAGCAP accredited subcontractor, the Offeror shall identify how quality control will function between organizations, and how data will be delivered to the government.
f. Describe how the Offeror would simultaneously manage multiple projects of varying size, complexity, locations, and accommodate unexpected surges if the need arises.
g. Describe your organization’s plan to manage risk, including identification of risk elements and risk mitigation strategies, and your plan to manage contract performance to ensure quality.
h. Offerors shall provide a narrative describing their corporate health and safety program, their familiarity with USACE, and other DoD, federal, and state health and safety regulations. The Offeror shall also provide any
OSHA recordable accident/incident involving explosives within the past five (5) years and identify safety risks that could occur during the evolution of this MATOC.
Factor 2 – Relevant Experience:
The Offeror shall provide relevant experience for at least three (3) but no more than six (6) recent and relevant projects on the Project Experience Form (Attachment 4). The Project Experience Form can be expanded (cell heights can be changed) to be multiple pages but is not exempt from the total page count of Volume I. Additional pages may be submitted for this factor that are not on the form but can only cover projects that have been submitted on the form.
Offerors shall choose projects which they have performed as the Prime or major subcontractor or projects that were completed by a major subcontractor depicted in the proposal. Offerors shall choose projects which demonstrate experience in project management and execution of work similar to that described RFP Attachment 1 – MATOC
PWS of this solicitation. Only projects that were physically completed or accepted by the customer within five years of the closing date of this solicitation, or projects which are nearing completion will be considered. For purposes of this solicitation, a project 75% complete or greater at time of proposal submission will be considered near completion.
The projects selected should show diversity in the types of work accomplished by the firm, demonstrate experience with different phases of the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA) process, and the federal Endangered Species Act (ESA), experience with firm fixed price contracts, teaming arrangements, key personnel and their roles on the project, regulatory information, in addition to basic cost information. Small arms range projects will not be accepted as demonstration of MMRP capabilities. Additionally, projects with regulatory interaction in the states of California, Utah, and Nevada will be viewed more favorably than regulatory interaction with other states.
All fields of the Project Experience Form must be clearly completed. If the Offeror was the Prime contractor, the
Offeror shall also describe its primary role/duties in execution of the work (i.e. the major components of the project which were completed by the Prime’s staff and major components which were subcontracted out). If the firm represented on the form was a subcontractor, the Offeror shall describe the primary role/duties in execution of the work the subcontractor was subcontracted to perform. The Offeror shall also provide information on the project as to the size, complexity, and distinctive and/or unique features of the project.
Factor 3 – Key Personnel:
For the MATOC evaluation only, the contractor shall submit resumes for the following key personnel: Program
Manager, Contractor Quality Control Manager, Project Manager, Safety and Health Manager, Quality Control
Geophysicist. Only one resume per position will be accepted. Each resume shall support all qualifications described in RFP Attachment 1 – MATOC PWS, Part 5. These five (5) key personnel will be the only personnel considered for this factor and their five resumes will be the only resumes required in proposals. Resumes of key personnel from a selected awardee of a MATOC may be requested by the KO prior to award and would include any or all of the key personnel identified in Part 5 of RFP Attachment 1. Similarly, resumes of key personnel of selected awardee for TO
0001 may be requested by the KO prior to award of the TO. Resumes would be checked and used to approve key personnel for the MATOC and/or TO. TO key personnel may be different than those used for the MATOC including those submitted for evaluation as part of Factor 3 (such that the project manager whose resume is submitted for
Factor 3 does not have to be the project manager proposed for TO 0001.
The resumes should clearly display proposed job title, education, professional registrations including the states in which the individuals are registered, special qualifications and experience record showing title of position, specific duties, responsibilities, and assignments and the dates these were held within the last five (5) years. In addition, the
Offeror shall provide resumes that demonstrate ability to perform Military Munitions Response Program (MMRP) projects within the scope of the MATOC PWS.
Note: After contract award, a change in any key personnel from what was proposed will need approval from the
Contracting Officer.
Factor 4 – Technical/Risk Approach to Fort Douglas Impact Area MRS (Seed Task Order):
The intent of the Government is to award a Task Order (TO) for Remedial Investigation, Feasibility Study, Proposed
Plan, and Decision Document of Impact Area Munitions Response Site (MRS) at Fort Douglas Formerly Used
Defense Site, Salt Lake City, Utah as outlined in Section M of this solicitation, to an IDIQ contract holder of this
MATOC based on Best Value Trade-off procedures. This factor will be the sole technical factor used to select the
TO awardee from the MATOC holders and will be significantly greater than price. The Offeror shall submit no more than 20 counted pages detailing the proposed technical approach for completing all tasks required by the Seed Task Order PWS. The following shall be covered in the proposal for Factor 4.
a) Demonstrate technical understanding of the work requirements, including, but not limited to:

ii) Quality control processes needed to show confidence in data and clearly differentiating when different technical approaches will be used and how data will be verified and validated. Potential for data gaps using the primary approach has contingent (alternative approach(es)) as needed.
iii) Geophysical approach will provide digital mapping of lateral and vertical extent, differentiating high and low munitions use areas.
iv) Chemical sampling and analytical approach will provide detail for mapping lateral and vertical extent of munitions constituents (if present).
v) Activities required for protection of cultural and environmental resources.
vi) Approach is anticipated to provide sufficient quality and quantity support the further action to be developed as part of this contract (to include the risk assessment, CSM updates, alternatives analysis, if needed, and the
Proposed plan and Decision Document), as well as any future remedial action, if needed.
vii) Descriptions of key deliverables and appropriate documentation (including Uniform Federal Policy-Quality
Assurance Project Plan (UFP-QAPP), utilizing the phased MR-QAPP approach and explanation of the data usability assessment at the end of each phase).
viii) Approach for public engagement and coordination demonstrates understanding of processes to support execution of work and develops understanding of land use scenarios for the risk assessment
ix) For approaches that include innovative technology:
(1) Technology description including spec sheets
(2) References for validation studies, technical papers or any other documentation for the usability and verification of the technology/approach
(3) Qualitative and quantitative impact on specific data objectives if innovative approach is used
(4) Incorporation of site-specific characteristics and limitations on innovative approach
(5) Quality control processes needed to show confidence in data collected using innovative approach, and
(6) Activities required for protection of cultural and environmental resources as related to the innovative approach.
b) Include an activity-based work schedule that outlines activities and identify milestones tasks defined at the appropriate detail level and logically sequenced to support and manage completion of the performance objectives for both sites using the technical approach identified herein. All base and optional tasks shall be included to show planned task duration and interdependency. Optional tasks should be identified as optional. If innovative technology is included as part of the approach, clearly show planned duration of all tasks involving innovative technology and their interdependency with other tasks in the approach. For proposal purposes, assume Task Order award date of around August 15, 2021. NOTE: Recommend a flow chart to show how tasks involving innovative technology relate to other tasks and identifies decision points that will be used to ensure all objectives are achieved.
c) Present an organizational chart which includes all Key Personnel and shows lines of communication and lines of authority. If the Offeror is a joint venture with, or is in a teaming agreement with a DoD Advanced
