1.4.7.1 Lake Level Fluctuation
Lake levels fluctuate regularly on Folsom Lake. Contractor to obtain and assess historical operational information for lake level fluctuation at http://cdec.water.ca.gov/cgi-progs/selectQuery?station_id=FOL . The station ID code is FOL. The Contractor is to ensure that work is planned and conducted in a manner consistent with the historical lake level fluctuation. The Government operates the lake level at full pool elevation of 468.34 feet on June 1 of each year, falling over the summer until October 1 when the reservoir elevation falls to maximum elevation
428.34 feet no later than November 17 to create flood storage. A maximum elevation of 428.34 is held until March 1 when the reservoir rises but does not reach elevation 468.34 earlier than June 1. The contractor is cautioned that storm events may cause rapid lake level rise above the prescribed Water Control Manual elevations described above. Water levels above the prescribed elevations result in reservoir releases in accordance with Water Control Diagram to bring the water level back to a level prescribed for that date. Contractor assumptions of lake levels lower than the operated levels described above are at Contractor's sole risk.
Prior to the start of construction, the Contractor shall prepare and submit for approval a water level rise action plan that addresses how the contractor proposes to relocate equipment and material stockpiles should a lake level rise above elevation 468.34 feet NAVD88 be predicted. This plan shall be kept in place throughout the duration of the construction contract. This plan shall also include provisions for dewatering of staging areas, stockpile areas, haul road, or project excavations that become inundated by rising water levels. The Contractor shall be prepared to perform any necessary dewatering. No excavations or other work will be allowed on the upstream side of Dike 8 if pool levels are above elevation
468.34 feet NAVD88 or forecasted to rise above this elevation, without prior approval by the Contracting Officer. Best management practices shall be employed by the Contractor in accordance with the Contractor's approved SWPPP (see Section 01 57 23 TEMPORARY STORMWATER POLLUTION CONTROL) and runoff from the site shall meet all State, local, and federal regulations.
Furthermore, the water level rise action plan shall include a contingency plan with proposed measures to protect the downstream areas which have a reduced level of protection due to construction activities and to restore the dike crest to the original crest elevation. Requirements for restoring the dike crest elevation are provided in Section 35 73 13 EMBANKMENT FOR EARTH DAMS. The plan shall include lake level monitoring, lake inflow rates at which the plan will be activated, material and equipment to be used in performance of the contingency plan, and the existing location, type and quantity of the stockpiled emergency material. The Contractor shall keep a sufficient quantity of embankment fill material on the project site for the duration of the construction period, protected from inclement weather, for use as emergency backfill as necessary.

If escorted, it will be for 100% of the time while within the project perimeter by a Reclamation-badged individual, whose sole duty at the time is as an escort service. Escorted personnel shall be provided a visitor's badge, to be worn at all times. Twenty-four (24) hour advance notification will be provided to Reclamation's security force by the Contractor's Security Manager and will include the name of the company, the name and driver's license number of the individual(s), the purpose of the visit and the name and phone number of the sponsoring Federal agency, contractor or subcontractor on site. Entry may be refused without this notification. For Security Zone 1 only, unescorted suppliers, vendors and visitors shall be issued (and shall display) a contractor-issued visitor's badge. The individual's name, company, driver's license number, state, employer's phone number, POC on site, purpose of visit, and make and model of vehicle (if applicable) will be recorded before access is permitted on site. The individual and vehicle are subject to search and/or inspection by the Reclamation's security forces.
1.5.6.4 Loss of Access Badges
The Contractor's Security Manager shall notify Reclamation immediately if any access badges are lost or stolen. Failure to report a lost badge may result in denial of a replacement badge.
1.5.6.5 Misuse of Access Badges
Any contractor personnel who misuse an access badge, enter an unauthorized Security Zone, provide the badge to others, follow improper escort procedures, or other misuses face the following actions:
a. First offense - warning and requirement to retake the Folsom Security Training.
b. Second offense - permanent loss of access badge.
1.5.6.6 Personnel Entry Approval
Delay or failure to obtain entry approval will not affect the contract price or time of completion.
1.5.7 Personal Identification Verification Identification
Contractor employees needing to undergo a Personal Identification Verification Identification (PIV-ID) full background check shall be provided the appropriate background investigation forms. After the background investigation forms are completed, the contractor employee shall be required to appear in person before a Government PIV-ID Registrar to submit the background investigation forms, have personal identity verification documents verified, have fingerprints and photograph taken, and sign the PIV-ID Badge Request Form. The Contractor must make their personnel available at the time and place specified by the Government in order to initiate this process. The following forms shall be used to initiate the background investigation and PIV-ID process: OPM SF 85P, Questionnaire for Public Trust Positions; OF 306, Declaration for Federal

Employment; FD-258, Non-Federal Employee Fingerprint Form; Fair Credit Reporting Act Authorization Form; and PIV-ID Badge Request Form (paper or electronic). The cost for completing and submitting the above forms and the cost of the background investigation shall be borne by the Contractor.
Contractor employees are required to give, and to authorize others to give, full, frank, and truthful answers to relevant and material questions needed to reach a suitable determination. Refusal or failure to furnish or authorize provision of information may constitute grounds for denial or revocation of credentials. The Government will conduct a background investigation for any individual that will be issued a PIV-ID Badge. The level of background investigation for each contractor employee will be determined by the Government based on the risk and sensitivity levels.
Each employee in a position designated as higher than Low Risk Non-Sensitive shall be reinvestigated on a periodic basis. If the final adjudication is unfavorable on a contractor employee, remove the individual from the contract and access to the project site. Upon completion of a favorably-adjudicated background investigation, contractor employees will be issued a Government PIV-ID Badge. When the PIV-ID Badge is printed, each employee shall be required to appear in person before the PIV-ID Badge Issuer for identity verification, to sign the PIV-ID Badge Request Form, and receipt of the PIV-ID Badge.
1.5.8 Vehicle Access Requirements
All vehicles, including personal vehicles and delivery vehicles, are subject to search by the Reclamation security force. Contractors need to be aware that changing Homeland Security threat levels may require the introduction of random or continuous searching of vehicles entering the work site. All vehicles entering Critical Areas and temporary Critical Areas are subject to a thorough Reclamation security inspection that could last up to 15 minutes. Once a vehicle has entered a Critical Area and passed security inspection, it will be subject to Reclamation random daily inspection only, unless there is cause or concern for additional inspections, such as if the vehicle has left the site for an extended period. All contractor employees driving on government property are required to have in their possession a valid picture identification issued by the Department of Motor Vehicles.
1.5.8.1 Personal Vehicles
No personal vehicles are allowed on top of the dams and dikes (Security Zone 3). Personal vehicles allowed to remain on site must display a current registration certificate.
1.5.8.2 Contractor Vehicles
All construction equipment shall be pressure washed to remove all mud, soil, and plant/animal material before bringing the equipment on site to limit the introduction of noxious weeds. Any contractor trucks or construction equipment requiring access on top of the main dam or dikes require Government approval. Approval includes proof of a current registration certificate, which must be displayed on the vehicle at all times when on site.
The contractor is responsible for making sure that all construction equipment and delivery vehicles, including haul trucks, are free of all mud, soil and debris when leaving the site and entering the public highway. Required tracking control best management practices are provided in Section 01 57 23 TEMPORARY STORM WATER POLLUTION CONTROL.
SECTION 01 00 00 Page 15
1.5.8.3 Delivery Vehicles

Notify the Contracting Officer if any construction activity will potentially interfere with operation of Folsom Dam.
1.7 WRITTEN GUARANTEES AND GUARANTOR'S LOCAL REPRESENTATIVE
Prior to completion of the contract, obtain and furnish to the Contracting Officer's representative written guarantees for all the equipment and/or appliances furnished under the contract. Furnish with each guarantee:
The name, address, and telephone number of the guarantor's representative nearest to the location where the equipment and/or appliances are installed, who, upon request of the Using Service's representative, will honor the guarantee during the guarantee period and will provide the services prescribed by the terms of the guarantee.
1.8 PRICING OF CONTRACTOR-FURNISHED PROPERTY
At the request of the Contracting Officer, promptly furnish and must cause any subcontractors to furnish, in like manner, unit prices and descriptive data required by the Government for property record purposes of fixtures and equipment furnished and installed by the Contractor.
1.9 NOISE STANDARDS
The Contractor is responsible for providing adequate hearing protection as may be required for Contractor's personnel at the job site.
1.9.1 Exterior and Interior Noise Standards
Exterior and interior noise standards shall be as set forth in the Folsom Municipal Code, COF Muni Code , which is located on the city's website at https://www.folsom.ca.us/clerk/city_clerk/municipal_code.asp . Click on "Folsom Municipal Code" near the top of the page. In the left navigation pane, click "Title 8" and "Chapter 8.42" for "NOISE CONTROL." Noise level standards, as set forth in the following table, are measured at any affected single- or multiple-family residence, school, church, hospital or public library. The Government will strictly enforce these noise standards.

1.9.2 Noise Source Exemptions
Noise sources associated with construction, provided such activities do not take place before 7 a.m. or after 6 p.m. local time on any day except Saturday or Sunday, or before 8 a.m. or after 5 p.m. local time on Saturday or Sunday, are exempt from the above standards.
1.9.3 Noise Measurement Criteria
Noise measurements shall be made with a sound level meter using the A-weighted network (scale) at slow meter response. Fast meter response shall be used for impulsive type sounds. Calibration of the measurement equipment utilizing an acoustical calibrator shall be performed immediately prior to recording any noise data.
The exterior noise levels shall be measured within 50 feet of the affected residence, school, church, hospital or public library. Where practical, the microphone shall be positioned 3 to 5 feet above the ground and away from reflective surfaces. The interior noise levels shall be measured within the affected dwelling unit, at points at least 4 feet from the wall, ceiling or floor nearest the noise source, with windows in the normal seasonal configuration. The reported interior noise level shall be determined by taking the arithmetic average of the readings taken at the various microphone locations.
1.9.4 Variances
Pursue any variances directly with the City of Folsom Public Works Director/City Engineer.
SECTION 01 00 00 Page 17
1.10 UTILITIES
Immediately notify the Contracting Officer in writing if utilities not shown on the plans and not visible as to the date of this contract are encountered within the construction limits of the entire project, and such utilities will interfere with construction operations, to enable a determination by the Contracting Officer as to the necessity for removal or relocation. If such utilities are removed or relocated as directed by the Contracting Officer, an equitable adjustment in the contract price must be made pursuant to the Contract Clause CHANGES to cover the cost of any additional pertinent work or delay.
It is anticipated that pole lines, signs, pipelines and private improvements that would interfere with or are to be replaced by new construction will be removed to new locations by the owners (or by the State of California or its agents) in advance of construction operations.
Notify the Contracting Officer at least 30 calendar days in advance of the date on which work will be started requiring the removal of such utilities or private improvements. Care must be taken to preserve and protect any such improvements from injury or damage during construction operations.
Assume full responsibility for reimbursing the owners for any damage to their properties, utilities, or improvements, or interference with their services caused through construction operations. Should such damage be found to have been caused without the Contractor's fault or negligence an equitable adjustment in the amount due under the contract will be made under the applicable CONTRACT CLAUSES.
1.11 PLANNED UTILITY OUTAGES AND STREET CLOSURES
All utility outages and street closures must be of as short a duration as possible and must be scheduled as far in advance as possible with the Contracting Officer, in no case less than 14 days before the outage or closure. Obtain from the Contracting Officer in writing a statement or schedule giving the permissible times of outages or closures for particular installations and the maximum time allowed for each. Strictly observe such schedules and the contractor will be held responsible for any violations.
1.12 TIME EXTENSIONS FOR UNUSUALLY SEVERE WEATHER
This provision specifies the procedure for the determination of time extensions for unusually severe weather. In order for the Contracting Officer to award a time extension under this clause, the following conditions must be satisfied:
a. The weather experienced at the project site during the contract period must be found to be unusually severe, that is, more severe than the adverse weather anticipated for the project location during any given month.
b. The unusually severe weather must actually cause a delay to the completion of the project. The delay must be beyond the control and without the fault or negligence of the Contractor.
The following schedule of monthly anticipated adverse weather delays is based on National Oceanic and Atmospheric Administration (NOAA) or similar data for the project location and will constitute the base line for monthly weather time evaluations. The Contractor's progress schedule must reflect these anticipated adverse weather delays in all weather dependent
