HOW TO SUBMIT ACCESS REQUESTS
Access requests must be sent to Daniel Czech using the instructions on FBO..
The body of the email should have a brief description of the purpose of the visit.
CAC HOLDERS ----- A completed MOTCO Form 190-3 must be attached.
NON CAC HOLDERS ----- A completed MOTCO Form 190-3 and NCIC Consent Form must be attached.
Do not send incomplete packets. For example: Do not send a MOTCO Form 190-3 with 10 names listed and 8 NCIC Consent Forms. This will automatically be returned for corrections.
Only complete packets will be submitted. A complete packet contains the MOTCO Form 190-3 and NCIC Consent.
MOTCO FORM 190-3 (Required for ALL visitors)
The MOTCO Form 190-3 is the template used for requesting access onto MOTCO. This form is in the same format as the “Master Access Roster” that contains individual profiles. The file is locked against format changes, to keep the integrity of the Master Roster. DO NOT CHANGE THE TEMPLATE. Copying and pasting will disrupt the formatting. If the format has excessive changes requiring extra effort to change back, the request will be returned to sender to be fixed.
Company Name: The Company the visitor works for.
Reason for Access: Why are they accessing MOTCO.
Sponsor: The name and contact information of the person who will be sponsoring the visitor. (Must be a CAC holder)
Frequency: Tells the officer at Pass and ID what hours the visitor is allowed on base. Here are a few examples:
----- Day passes as needed// M-F // 0700-1600 This tells the Pass and ID officer the visitor will not come on a daily basis, but is anticipated to visit monthly or quarterly. Upon arrival at Pass and ID the individual will tell the officer how many days they are expecting to be on base. “DAY PASSES AS NEEDED” WILL ALLOW THE VISITOR TO RECEIVE PASSES FOR UP TO TWO WEEKS.
----- Day passes as needed// M-F // 0700-1600 (Example - Call John Smith at Ext 4400 to confirm) This would be used to make sure you always know when your visitor will be on base if they are not frequently coming on base.
----- Daily// M-F // 0700-1600 – Less than 6 months This person will be working on the base on a regular basis. “DAILY (>6 MONTHS)” PASSES CAN BE ISSUED UP TO 30 DAYS AT A TIME.
----- Daily// M-F // 0700-1600 – More than 6 months This person will be working on the base on a regular basis. “DAILY (<6 MONTHS)” DBIDS CARD WILL BE ISSUED.
----- Blank or Unclear block will be defaulted to "One day passes only// 0700-1600" in the database. The visitor will have to come into Pass and ID every day to get a pass until an updated MOTCO Form 190-3 is resubmitted.
24/7/365 access requires justification. This gives a better indication as to what type of pass will be issued.
MOTCO FORM 190-3 (Required for ALL visitors) CONTINUED
Last Name: The visitor's last name.
First Name: The visitor's first name.
Middle Name: The visitor's middle name. IF NO MIDDLE NAME, ENTER NMN (No Middle Name)
DOB: Enter the visitor's date of birth (enter it like this “6/20/83” it will convert to the correct date format).
DL#: Enter the visitor's driver's license number.
DL State: Enter the state the visitor's driver's license was issued in.
Access From: The date access is requested to begin. Please enter the access date as a future date, not a date that has already passed (enter it like this “6/20/83” it will convert to the correct date format).
Access Thru: The date access is requested to end. If requesting one year of access, it would look like this: 22-Mar-2015(Start date) 21-Mar-2016(End date) (enter it like this “6/20/83” it will convert to the correct date format).
CAC: Enter "Y" if they have a CAC; Enter "N" if they do not
Tidal: Put an "X" for Tidal access ----- Leave blank if not needed
Inland: Put an "X" in Inland for Inland access ----- Leave blank if not needed
Foreign Born: Put an “X” if foreign born ----- Leave blank if not (Do not be alarmed if you put an “X” in it and it turns YELLOW- This is meant to stand out that additional actions will be needed)
Escort Required: Put an “X” if escort required ----- Leave blank if none needed (Do not be alarmed if you put an “X” in it and it turns RED- This is meant to stand out that additional actions will be needed). Remember, as the sponsor, you are acknowledging and taking responsibility for their actions on base.
Once a person is vetted, it is valid for one year. They will not be re-vetted unless 10 months have passed (in the event they need to have their access extended beyond a year of being vetted).
NCIC CONSENT FORM (Required only for visitors who do not have a CAC or other Federal Government ID Card)
All information MUST be legible on this form. If it cannot be read, it will not get processed.
*** Common mistakes
*** I (print full name; first, middle, last): PRINT the visitor's full name; First, Middle, Last ---- It must be in that order.
Signature of Requestor: This is where the individual requesting access signs.
Race: Enter the visitor's race (White, Black, Hispanic…).
Date: Enter the date the form is being signed.
Date of Birth: Enter the visitor's date of birth.
*** Place of Birth (City and State): Enter the city and state of the visitor's place of birth. This is very important and the main reasons forms are returned for correction. If the place of birth is outside the United States it also must contain a city. For example: Tijuana, Mexico
Gender: Enter the visitor's gender.
Social Security Number: Enter the visitor's Social Security Number.
Address: Enter the visitor's CURRENT address.
Company Name: Enter the visitor's company name.
Telephone: Enter the visitor's contact information.
It is recommended you type the information on the NCIC and then have the visitor sign the document.
If you send a NCIC form that is not properly filled out, changes cannot and will not be made to it. LA, CA could almost positively be assumed as Los Angeles, CA, but if it is not spelled out, it will get returned. The NCIC form is authorization to perform a criminal background check utilizing the Federal Bureau of Investigation (FBI) criminal database. Sponsors are not authorized to make alterations to a signed document. They will be held legally liable for the information it contains and the results.
Remember the sponsor is required to review and proofread the request PRIOR to sending it. Errors cause delay. Best practice: Send it right the first time.
FOREIGN BORN
MOTCO is designated as a restricted area in accordance with AR 190-13. Under the auspices of the AR 190-13, para 6-2, Homeland Security Presidential Directive (HSPD)- 12, Directive-Type Memorandum 09-012, and Army Execution Order (EXORD) 033-15, the Installation Commander has restricted access to U.S. Citizens conducting official business on MOTCO.
Acceptable forms for citizenship verification:
· Consular Report of Birth Abroad, or FS-240, provides proof of citizenship if the birth was registered at the nearest U.S. consulate when the individual was born. (Usually people born to military families PCS'd overseas).
· Naturalization Certificate or Certificate of Citizenship
· US Passport
Foreign born individuals must present an original or copy with a raised embossed seal/valid proof of citizenship upon arrival to Pass and ID. The document number and expiration date will be loaded in the profile in the Master Access Roster. Once the information has been recorded, the individual should not need to present the documentation again. It is however recommended that the individual brings the documentation with them the next time they come to Pass and ID in case the information was not uploaded properly. The Pass and ID officer will notify the visitor that their information has been properly uploaded.
*****Citizenship in the following countries WILL NOT require proof of US Citizenship:
Australia, Canada, New Zealand, and the United Kingdom (Wales, England, Scotland, Ireland).
TRANSPORTATION WORKER’S IDENTIFICATION CARD (TWIC)
TWIC used to be an accepted form of ID that didn't require vetting. However, the DoD has changed their stance on the TWIC, due to the fact it is only vetted against the terrorism database and not the NCIC III database (criminal history database). The terrorism database is comprised of personnel who have known connections or interactions with terrorist organizations and the NCIC database tracks all arrests and criminal activity within the last 99 years which contains the information concerning the 11 factors we vet against to decide access or no-access.
REAL ID (EXORD 110-16- REAL ID)
Effective 10 Jan 16, driver licenses and ID cards from the states of Illinois, Minnesota, Missouri, New Mexico, Washington and the Territory of American Samoa do not comply with the Real ID Act and can no longer be used as the sole source for identity proofing for unescorted access to military installations and facilities.
Visitors that possess driver licenses and ID cards from these states will need to provide an acceptable secondary proof of identification prior to be allowed access.
Page 5-6 of the EXORD for a list of acceptable identification for identity proofing.

• DHS "TRUSTED TRAVELER" CARDS (GLOBAL ENTRY, NEXUS, SENTRI, FAST)
• TWIC (TRANSPORTATION WORKER IDENTIFICATION CREDENTIAL)
• MERCHANT MARINER CARD ISSUED BY DHS/UNITED STATES COAST GUARD (USCG)
• DRIVERS LICENSE ISSUED BY THE U.S. DEPARTMENT OF STATE
• BORDER CROSSING CARD (FORM DSP-150)
• U.S. CERTIFICATE OF NATURALIZATION OR CERTIFICATE OF CITIZENSHIP
• EMPLOYMENT AUTHORIZATION DOCUMENT WITH PHOTOGRAPH ISSUED BY THE DHS (FORM 1-766)
• IDENTIFICATION CARD ISSUED BY FEDERAL, STATE, OR LOCAL GOVERNMENT AGENCIES, PROVIDED IT CONTAINS A PHOTOGRAPH AND BIOGRAPHIC INFORMATION SUCH AS NAME, DATE OF BIRTH, GENDER, HEIGHT, EYE COLOR, AND ADDRESS.
ESCORT PRIVILEGES
Escort privileges will be granted after receiving a written request from the government sponsor.
Requests should include:

After request has been sent, the individual receiving escort privileges will proceed to Pass and ID to sign the Escort Responsibilities form, and get an updated pass authorizing escort privileges.
----- Escorting privileges are not provided to circumvent the vetting process and bring contractors on the installation to complete day jobs.
----- Side-by-Side accompaniment is required by the individual providing the escorting services while on MOTCO.
----- “Escorting” means – ensuring the escorted individual(s) are continually accompanied while with-in a restricted area in a manner sufficient enough to observe whether the escorted individual is engaged in activities other than those for which escort access was granted.
----- If the escort is found in violation of their escorting responsibilities they will be subject to arrest, permanent barring, and prosecution under federal statutes.
----- MOTCO reserves the right to revoke privileges at any time.
REASONS FOR DENIAL
In accordance with Army Directive 2014-05, these are the 11 disqualifying factors:
1. The NCIC III contains criminal arrest information about the individual that causes the senior commander to determine that the individual presents a potential threat to good order, discipline, or health and safety on the installation.
2. The installation is unable to verity the individual's claimed identity based on the reasonable belief that the individual has submitted fraudulent information concerning his or her identity in the attempt to gain access.
3. The individual has a current arrest warrant in NCIC, regardless of the offense or violation
4. The individual is currently barred from entry or access to a Federal installation or facility.
5. The individual has been convicted of crimes encompassing sexual assault, armed robbery, rape, child molestation, production or possession of child pornography, trafficking in humans, drug possession with intent to sell or distribute.
6. The individual has a U.S. conviction for espionage, sabotage, treason, terrorism or murder.
7. The individual is a registered sex offender.
8. The individual has a felony within the past 10 years, regardless of the offense or violation.
9. The individual has been a convicted of a felony firearms or explosives violation.
10. The individual has engaged in acts or activities designed to overthrow the US Government by force.
11. The individual is identified in the TSDB as known to be or suspected of being a terrorist or belonging to an organization with known links to terrorism or support of terrorist activity.
CAMERA PASSES
Camera passes are required for ANY (employee, contractor, visitor…) individual who wishes to take photos for OFFICIAL purposes. Due to the sensitive nature of MOTCO’s mission, taking photos for unofficial use is forbidden.
A camera pass must be requested through a government sponsor. Once the request is filled out, the sponsor will send it, and a justification, to the Pass and ID email list (usarmy.motco.sddc.list.passandid@mail.mil). Upon approval, the individual will come to Pass and ID to get the documentation that will allow for them to take photos.
Cameras are subject to inspection at any time by MOTCO Department of the Army Civilian Police (DACP), Department of the Army Security Guards (DASG), and sponsor.
Photos posted to any social media platform without prior approval from the Installation Commander are prohibited.
The following locations require express written approval for taking photos:
Piers
Cranes
Vessels
Access Control Points (ACP)
Entry Control Points (ECP)
Perimeter Fences
Easement Entry Points
Power Stations
Power Sources
Any other mission critical infrastructure
EXCEPTIONS
----- Deliveries where the driver’s information is not available prior to arrival will be processed as a walk-in. Depending on the workload of Pass and ID when the visitor arrives will dictate how long of a wait the walk-in will take to process. If you know you have a delivery coming – let Pass and ID know. It will save a lot of time from trying to track down the point of contact.
----- Emergency work orders where the technician’s information is not available prior to arrival will be processed as a walk-in. Depending on the workload of Pass and ID when the visitor arrives will dictate how long of a wait the walk-in will take to process. If you know you have a delivery coming – let Pass and ID know. It will save a lot of time from trying to track down the point of contact.
