AMENDMENT 01

STATEMENT OF WORK

UTAH AIR NATIONAL GUARD/151 ISRG

PURCHASE AND INSTALLATION OF STANDING DESKS

Utah Air National Guard, 151st ISR Group, 765 North 2200 West, Salt Lake City, Utah 84116-2999

1.0 INTRODUCTION

1.1 The Utah Air National Guard has a requirement to purchase and install new workstations at the Roland R. Wright Air National Guard Base, Building 302, mission floor.

2.0 TECHNICAL REQUIREMENTS

2.1 Scope of Work: This Statement of Work (SOW) identifies the work that shall be performed by the contractor for the purchase and installation of new workstations at the Wright Air National Guard Base, Building 302, mission floor. The contractor shall provide all equipment, tools, supplies necessary for the proper delivery and installation of all requirements within this SOW.

Equipment:

27 standing desks with cable support assembly and bracket 27 monitor support assemblies 27 chairs 27 cable management solutions (trough) Cubicle dividers for each row of desks

Desks:

The desk dimensions will be a minimum of 42"W x 30"D and will be adjustable from sitting to standing

The desk surface shall be of high quality laminate top with thermo edge or solid wood The desk adjustment mechanism will be motorized and capable of lifting the desk surface, monitor stand, four 27” LCD displays (21” x 14 ½ “ x 3”), keyboard, mouse, and headset, supporting a minimum of 250lbs

Up and down controls shall be electronic, with easy to use, intuitive controls.

Desks shall be freestanding Sitting height of the desk surface will be at most 29” above floor level. Standing height of the desk surface will be at least 48” above floor level.

An adjustable rack mounted docker shall be provided by the contractor to mount computer equipment for each workstation providing minimum of 3U and 30 inches of depth mountable space while providing ease of access to the equipment to the front and back of the equipment, and in such a way that does not interfere with the movement of the desk or compromise the integrity of the fiber and cables. Equipment shall be securely fastened with screws. Minimum size requirements are 19” W x 36” D x 5.25” T

Each workstation has cables and fiber coming into the mounted equipment (movement must be minimized) and cables and fiber coming out of the equipment must be protected and secured, providing protection from pinching, binding, and chaffing, in raceways or trays from equipment all the way into the removable floor tiles. The raceways or tray must be free of sharp edges. Each raceway or tray shall be completely enclosed providing protection from every angle while allowing quick vertical and horizontal access for servicing cable

Each desk shall contain an integrated power supply with a 20 Amp capacity with a minimum of six power receptacles to accommodate 4 monitors, server and CPU.

The entire console shall lift, to include CPU cabinets to prevent stretching of computer cables, fibers, and cords

The console shall not use computer software to control the height The power cords for the desks should be a minimum of ten feet long to accommodate the current floor power outlets (every ten feet in the floor)

Quad Monitor Support Assembly:

There will be four monitors on each desktop and will require a quad screen monitor assembly which will support 27” LCD displays Monitor mount will be attached to the desk surface so that the monitors will move with the desk providing vertical, horizontal and lateral movement, monitors will not hang over the edges of the desk surface in order to prevent interfering with adjacent desks

Monitor mount will provide ability to mount four 27 inch monitors A cable management solution will be provided to prevent damage to monitor and interface cables as the desk moves The contractor will install rigid dividers (Cubicle walls) between rows of desks

Chairs:

43” h x 28.25 w x 18.5” d (large size), ergonomic mesh chairs, fully adjustable arm, tilt limiter and seat angle, adjustable lumbar support, footrest, adjustable lumbar height, hard floor roll-away resistant casters, standard color.

Cubicle Wall Dividers:

Rigid dividers between each row of back to back desks, preferably seven feet high, but no less than five feet.

Dividers at the end of each row shall be constructed in a way to allow semi-blockage from the adjacent aisle way

Panels should be constructed of metal and covered with acoustic insulation material Fabric should be a tan or light brown material Panels should be installed in a way that will allow full access to electrical outlets on the floor Installation, trim, and mounting hardware included

Design:

The included design layout is a relative solution only, and actual installation and total number of seats and desks can vary depending on final layout which meets our minimum mission requirements and numbers provided

Removal of Existing Equipment:

The government will remove all computer equipment and cables The contractor shall disassemble and remove all existing desks and move them to a designated holding are as determined by customer

Other Information:

A standard commercial warranty shall be included that covers 24hr operations usage The 151 ISRG will provide appropriate escorts onto the base and secured facilities, which will be coordinated with 151st Security Forces, and the 151 ISRG Security Office.

The vendor assumes liability while shipping, transporting, and installing the desks, until completion of installation and the Government fully accepts the contract.

The vendor will ensure that all materials are shipped to a holding facility or warehouse prior to arriving on our base – nothing shall be direct shipped to our facilities.

3. DESCRIPTION OF TASKS – DELIVERABLES

Periods of Performance: 30 Days (includes lead time). Only 5 days (M-F) will be allowed for all removal and installation of equipment. The system outage will be coordinated with the 151st ISRG based on mission requirements.

3.1 Task 1 – The contractor shall order acceptable stand/sit adjustable desks, brackets, and monitor stands per specifications and measurements.

3.2 Task 2 – The contractor shall install the new desks, cable support assemblies and monitor support assemblies.

3.3 Task 3 – The contractor shall assemble and install all divider walls

3.4 Task 4 - The contractor shall assemble and set-up new chairs

3.5 Task 5 - Connect to power and function check all electrical components

3.6 Task 6 – The contractor shall return to function check desks and cable management once the government installs all workstations and hardware.

4.0 PERFORMANCE OBJECTIVE AND STANDARDS

Performance Requirement

Standard Surveillance Method Corrective Action for Unsatisfactory Performance

The contractor shall install new desks per design and specification

Installation will leave a fully functioning desk free of damage with full utilization of intended functions

Visual inspection by 151 ISRG technical personnel

Re-performance by the contractor at no additional cost to the government.

The contractor shall install monitor mounts and cable assemblies per design and specification

Installation will leave fully functioning mounts and cable assemblies free of damage with full utilization of intended functions

Visual inspection by 151 ISRG technical personnel

Re-performance by the contractor at no additional cost to the government.

The contractor shall assemble and install computer chairs per design and specification

Assembly and installation will leave fully functioning chairs free of damage with full utilization of intended functions

Visual inspection by 151 ISRG technical personnel

Re-performance by the contractor at no additional cost to the government.

5.0 INSPECTION AND ACCEPTANCE

5.1 The finished product will be inspected by the Contractor and Functional Area Chief (FAC) for compliance with contract objectives.

5.2 Certification by the Government of satisfactory equipment with installation is contingent upon the contractor performing in accordance with the terms and conditions of the referenced contract. When an observation indicates performance requirements are not being met, the FAC will notify the contractor and the Contracting Officer. The contractor will take steps to rectify the situation and notify the Government upon completion. Government reserves the right to require re-performance at no additional cost. Any action taken by the contracting officer as a result of surveillance will be in accordance with the terms of this contract.
