PERFORMANCE WORK STATEMENT

April 18, 2019

BASE FIRE ALARM NOTIFICATION,

DETECTION AND SUPPRESSION SYSTEMS

INSPECTION & TESTING

SW-1 SCOPE:

1.1 The project site is located at the 125th Fighter Wing Jacksonville International Airport, Duval

County, Florida. This project involves the inspection and testing of all base Fire Alarm, Detection and Suppression systems in buildings; 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 12, 13, 18, 19, 20, 22, 23, 25, 27, 28, 29, 30, 31, 35, 36, 38, 39, 40, 41, 42, 44, 45, 49, 50, 54, 55, 60, 61, 1111, 1407 and 1810 to include; Annual inspection and testing of all fire alarm panels for proper operation and supervision, smoke detectors, duct detectors, pull stations, audible / visual notification devices, and all supervisory devices for proper operation and annunciation

IAW with NFPA17,17A, 25, 72 and UFC 3-601-02 and UFC 3-230-02. This project also involves the inspection and testing of the Base Fire Suppression Systems to include the following systems. Wet (to include Hydrants, Water Tanks and Fire Protection Backflows), Dry sprinkler systems, Pre-action sprinkler systems, Dry chemical systems, Fire pump inspections, High Expansion Foam (HEF) Systems and Hood Systems. IAW with NFPA17, 17A, 25, 72 and UFC 3-601-02 and 3-230-02. Contractor shall provide an electronic copy of a detailed completed inspection report to include; list of all discrepancies found, required actions to render compliant, and include an itemized system repair quote for each facility.

Contractor shall provide a properly placed system certification tag on each inspected fire alarm panel and suppression systems IAW NFPA and UFC requirements. Contractor shall be NICET Level 2 qualified.

SW-2 SCOPE OF WORK:

2.1 All requirements must be met as they appear in this statement of work. This statement of work does not supersede any Federal Acquisition Regulation stipulations that may apply to this contract.

2.1.1 The Contractor shall provide supervision, transportation, labor, materials, equipment, fuel, and any other needed materials for the purpose of this project.

2.1.2 The Contractor is subject to all base fire, safety, security, and traffic regulations.

2.1.3 The Government contracts on the understanding that the Contractor is complying with applicable laws concerning workplace safety.

2.1.4 The Contractor is subject to all citations and penalties for violation of applicable workplace safety laws. The burden of compliance rests with the Contractor. Enforcement of workplace safety laws is the responsibility of the agency specified in the applicable law.

2.1.5 The Contracting Officer may notify the Contractor of hazards in Contractor operations that endanger Government personnel or property and require appropriate abatement action. This provision does not constitute assumption of liability for Occupational Safety and Health compliance on the part of the Government or negate other agency responsibility for enforcement of applicable laws.

2.1.6 Accident prevention principles must be thoroughly integrated into all operations. Unsafe conditions and practices, as identified by the Contracting Officer or designated representative, must be corrected immediately.

2.1.7 Any and all debris generated by the Contractor's operations shall be removed from the base. Disposal of all debris shall be in accordance with existing local, state, and federal regulations. The Contractor shall be responsible for any permit or fees associated with the use of off-base disposal locations.

2.1.8 The Contractor shall be liable for any loss, destruction, or damage, and deterioration to

Government property, including vegetation and landscaping, resulting from Contractor operations. Any damage to government or personal property shall be repaired or replaced to the Contracting Officer's satisfaction at the Contractor's expense.

2.1.9 All work under this project shall meet applicable County, State, Federal, Air National

Guard construction codes, regulations and policies.

SW-3 SPECIFIC ITEMS OF WORK:

3.1 INSPECTION: Contractor shall provide all labor and equipment to perform an annual Fire

Alarm and Detection systems inspections on buildings 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 12, 13, 18, 19, 20, 22, 23, 25, 27, 28, 29, 30, 31, 35, 36, 38, 39, 40, 41, 42, 44, 45, 49, 50, 54, 55, 60, 61, 1111, 1407 and 1810.

3.2 IAW with NFPA 17,17A, 25, 72 and UFC 3-600-02 UFC 3-230-02 to include: Test all Fire

Alarm panel circuits for proper operation and supervision.

Test all smoke detectors, heat detectors, duct detectors and remote LED associated with the duct detectors.

Test all manual fire alarm pull stations.

Test all Audible / visual fire alarm notification appliances.

Test all supervisory devices for proper operation and annunciation.

3.2.1 Wet Sprinkler System - annual inspection for the following bldgs.

3.3 Testing/Certification: Contractor shall provide an electronic copy of a detailed completed inspection report to include; list of all discrepancies found, required actions to render compliant, and include an itemized system repair quote for each facility. Contractor shall provide a properly placed system certification tag on all inspected systems IAW NFPA and

UFC requirements.

3.4 "The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the FLORIDA

AIR NATIONAL GUARD via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address:

http://www.ecmra.mil/. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through

September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2013. Contractors may direct questions to the help desk at: http://www.ecmra.mil.”

SW-6 PERMITS/CLEARANCES:

6.1 Construction Permit - Prior to commencement of work, the contractor shall report to the

Customer Service Section in the Base Civil Engineering office, Building 28, telephone (904)

741-7611, to complete a construction permit form and login the construction start date. The contractor shall identify other anticipated permits at this time to reduce lead times.

Information obtained from the construction permit will be used for Entry Authorization

Letters (EAL’s) and other permitting requirements. There is no fee for the permit.

6.2 Work Clearance Request (AF Form 103) - A clearance request (also known as digging permit) is required prior to any excavation work. Permits can be obtained from the

Customer Service Section at the Base Civil Engineering office. The permit normally takes two working days to coordinate utility clearances. There is no fee for the permit.

6.3 Security Clearance - The Florida Air National Guard base has 24-hour security and entry control procedures. Contractors will be briefed on as needed basis concerning traffic routing and work site security restrictions. Entry Authorization Letters (EAL) for contractors are required and will be obtained at either the preconstruction conference or when requesting a construction permit. Information required for EAL’s shall include individuals name, company name, driver’s license number, project or contract number and title of the project. EAL’s are provided by the Base Civil Engineer and approved by the

Chief of Security. Due to tightened security measures, submit a copy of the vehicle

Certification tag 1 each On completion of system testing registration for each vehicle that will be on base, and also a copy of each individual’s driver’s licenses that will be on the installation, at least two weeks prior to construction start. All contractor personnel will be issued a contractor badge, which will identify the area or areas that the contractor is approved to be working. Each identified contractor will ensure that the identified issued badge is properly displayed while performing work on this installation. Also, each individual who is issued a badge must maintain control of his or her assigned badge at all times.

6.4 Access and General Protection/Security Policy and Procedures. This standard language is for contractor employees with an area of performance within Government controlled installation, facility, or area. The contractor shall comply with all applicable installation/facility access and local security policies and procedures, which may be obtained from the Contracting Officer’s Representative (COR). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by the local installation’s Security Forces, Director of Emergency Services or local Security Office. The contractor shall ensure compliance with all personal identity verification requirements as directed by DOD, HAF and/or local policy. Should the Force Protection Condition (FPCON) change, the

Government may require changes in contractor security matters or processes.

6.5 For Contractors that do not require CAC, but require access to a DoD Facility and/or

Installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center

Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (AFI

31-101 and AFI 10-245), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.

Advanced Distributed Learning Service (ADLS). All contractor employees and associated sub-contractor employees assigned to work on a government site and/or requiring access to a

DOD network will be required to register with ADLS at commencement of services in order to complete the necessary training required of them initially and annually for the duration of the contract (i.e., DoD Information Assurance, OPSEC, Protecting Sensitive Information).

AT Level I Training. This standard language is for contractor employees with an area of performance within a Government controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access to Government

Installations, Facilities and Controlled Access areas shall complete AT Level I awareness training within 10 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. Certificates of completion for each affected contractor employee and subcontractor employee will be maintained by the

COR or Antiterrorism Representative. AT level I Awareness Training is available through

JKO by completing the CBT at http://jko.jten.mil/courses/atl1/launch.html (no CAC required).

6.6 Burn Permit: Obtain a burn permit prior to any welding processes. This permit can be obtained by contact the 125th Fire Department at 741-7636 or 741-7637.
