About this file
This is a Past Performance Questionnaire (PPQ) form used by the Naval Facilities Engineering Command (NAVFAC) and U.S. Army Corps of Engineers (USACE) to evaluate contractor performance on previous federal contracts. The form (PPQ-0, dated 9/30/11) is designed to be completed by contractors and their clients to document performance history for use in evaluating proposals for future contract awards.
The questionnaire consists of two sections: contractor-completed blocks that capture basic contract information (contractor details, contract number, type, dates, pricing), and client-completed blocks that provide performance ratings and narrative feedback. Contractors must provide their CAGE code, DUNS number, point of contact information, work role (prime, subcontractor, joint venture), contract type (firm fixed price, cost reimbursement, other), contract dates, original and final pricing, and project complexity level. Clients evaluate contractor performance across seven categories: Quality (technical data preparation, quality standards compliance, problem resolution, quality control program), Schedule/Timeliness (delivery compliance, resource utilization), Customer Satisfaction (end user satisfaction, contractor cooperation, responsiveness), Management/Personnel/Labor (on-site management, workforce hiring and retention, property control, expertise, small business utilization, multi-project capability, change management, overall leadership), Cost/Financial Management (pricing compliance, innovative cost reduction, invoice timeliness and accuracy, accounting systems adequacy, contract termination history, financial stability), Safety/Security (environmental maintenance, security compliance), and General (emergency response capability, contractual compliance, rehire potential, overall performance). Ratings use a six-point scale: Exceptional (E), Very Good (VG), Satisfactory (S), Marginal (M), Unsatisfactory (U), or Not Applicable (N). Clients must provide narrative comments addressing strengths, weaknesses, deficiencies, and performance risks. The government reserves the right to verify all information provided and may contact clients directly or through the offeror.
