If an offeror submits more than three (3) forms, the Government will only review the first three forms received.

b) Technical Forms shall be submitted for work performed (in full or in part) within five years from the closing date of the solicitation. If a past experience is submitted that was wholly performed outside of five years from the closing date of the solicitation, the Government will not consider it as part of the evaluation.

c) Offerors shall provide enough information in the Technical Forms to demonstrate experience working with equipment that produces H2S, also with Varec and Shand and Jurs equipment, electric actuators, Uni-Scrub, Vibration Limit Switches, electronic gauges, and iron scrubbing media, as a prime or major subcontractor.

d) If an offeror proposes to subcontract at least 10% of the cost of work for this requirement, the offeror may submit a technical experience form for the subcontractor; however, any forms submitted for a subcontractor will count towards the three form limit.

e) Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies. In many cases, these changes have taken place between conclusion of relevant past experience and this acquisition. To facilitate the Government’s evaluation, include in this proposal volume a "roadmap" describing all such changes in the organization of your company, if a Technical Experience Form is being submitted for a company with a name different from that of the offeror.
