FAR 52.212-1, Instructions to Offers
1. PROPOSAL SUBMISSION
The Proposal: This Request for Proposal (RFP) is being issued in accordance with Federal Acquisition Regulation (FAR) Part 13 Simplified Acquisition Procedures. The submission of the documentation must specify acceptance of the terms and conditions of the RFP and concurrence with the Performance Work Statement at the proposed firm fixed price.
Electronic Representations and Certifications (FAR 52.212-3) Representations and Certifications. Offeror(s) representations and certifications shall be completed online and at least annually required to update them. The URL in this line of text facilitates the ease access to the site: http://orca.bpn.gov.
Small Business Program Representations (Apr 2011). All offerors are required to complete and submit Small Offeror’s Representations and Certifications – Commercial Items electronically at http://orca.bpn.gov.
Written Documentation: All written documentation will be used to evaluate the proposal. Offeror shall submit the following written documentation no later than the specified date to the location specified herein. Any information not specifically requested below shall not be evaluated or used in any way. This RFP contains a Performance Work Statement and requires the contractor to submit firm fixed price quotation to provide the services outlined in the Performance Work Statement (Attachment 1).
The proposal shall consist of three sections: (1) Mission Capability (Elements A. Technical Approach; Element B. Management and Staffing; and Element C. Training Support Plan and (2) Past Performance and (3) Price proposal. The entire proposal is limited to 40 single-sided pages. The proposal may be submitted only by electronic mail to the following address (Note: A facsimile will not be accepted):

1. MISSION CAPABILITY: The offeror shall demonstrate a clear understanding of the nature and scope of the work specified in the PWS. Failure to provide a reasonable and complete quotation shall reflect a lack of capability to perform the work requirements.
2. PAST PERFORMANCE/EXPERIENCE. The offeror shall include relevant past performance history for the past three (3) years, as it directly relates to the work being procured under the RFP. The offeror SHALL NOT go back any farther than 3 years for the submitted data. To be considered relevant, the submitted work must be of the same or similar nature, size, and complexity to the work specified in this RFP. The submittal shall include the offeror’s rationale as to how the work cited is considered the same or similar in nature, size, and complexity to the work specified in this RFP. The offeror shall discuss how the work cited demonstrates a capability to perform the work specified in the RFP. While Government contracts are preferred, non- Government contracts may be used. The following information shall be provided on all contracts relevant to this RFP: 1) Contract Number and type (i.e., FFP, Cost Reimbursement, IDIQ, etc.); 2) Award Date/Completion Date/Contract type; 3) Price/Cost – original/awarded AND final (or projected final, if contract is current); 4) Delivery Schedule – original AND final (or projected final, if the contract is current); 5) Address and telephone number for the Point of Contact; 6) Name, telephone number, fax number and email address for the Point of Contact; 7) A copy of any cure notices/show cause letters received and outline of the corrective action; and 8) Documentation concerning any Termination for Default actions.
3. PRICE PROPOSAL: The RFP contains a Performance Work Statement and requires the offeror to submit a firm fixed price quote to provide the services outlined in the Performance Work Statement.
