SUMMARY OF CHANGES

SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS

The following have been added by full text:

AMENDMENT 0003

Amendment 0003:

1. The purpose of this amendment is to provide answers to questions received regarding the subject Request for Proposal.

Question 1. It is understood from the RFP that offerors are to complete Tab B of the pricing model in accordance with the Task Order information included in Appendix C. Should reference be made to the Task Order in the accompanying technical volumes? Should the offeror provide some explanation on how they would perform the Task Order? Or is this merely a pricing exercise?

Answer 1: Requirements for the Technical & Management Volume II can be found in Section L.5.2.2., E.

Question 2. It says in the RFP that "A Past Performance Questionnaire, Appendix A, shall be provided to each reference used as a basis of the Offerors past performance, for completion by the Offerors customer for direct submission to Jeremy Michaels, Contract Specialist (jeremy.d.michaels.civ@mail.mil), to be received prior to the RFP closing date" - does this mean that Appendix A needs to be submitted separately from the rest of the proposal package or should the completed forms be included in the proposal package as well?

Answer 2: Completed Past Performance Questionnaire forms do not need to be included in the proposal submission. In accordance with the instructions provided in Section L.5.2.2., F. Past Performance Questionnaires, Appendix A, are to be submitted directly to the Contract Specialist.

Question 3. Attachment 4 to the RFP is a DD254 form. But I can see no reference to this form in section L of the RFP which outlines the requirements for the proposal submission. Does this form not need to be included in the proposal submission package?

Answer 3. The DD254 is the DoD Contract Security Classification Specification and will become an attachment to any resulting Contract. This form does not need to be included in the proposal submission package.

1. As a result of this amendment the due date and time for responses to the RFP is not extended.
