Statement of Work
FOR
MONARCH VERTICAL MACHINING CENTERS
1.0 Scope:
Provide unlimited on-call emergency repairs and annual inspection/alignment for the Government owned Monarch Vertical Machining Centers models VMC150B and VMC-200B.
This equipment is located in Bldg. 145 and Bldg. 5 at Anniston Army Depot, Anniston, AL 36201- 4199. The equipment to be covered is SN: 2004501B and 2008901B. The maintenance coverage shall be for a one-year base period beginning with award of this contract, plus four (4) option years.
2.0 Maintenance Requirements:
The contract shall cover the cost of providing all things necessary to perform unlimited emergency repairs and annual inspection/alignment for the machining centers listed, including labor, tools, diagnostic equipment, transportation, schematics, documentation, and any other services to repair the Machining Centers. All repairs and annual inspection/alignment shall be performed by Contractor personnel that are experienced and highly trained in operating, maintaining, and servicing the equipment in accordance with the original equipment manufacturers (OEM) published procedures.
2.1 ANNUAL INSPECTION AND ALIGNMENT:
The Contractor shall provide the Government annual inspection and alignment in accordance with the OEM specifications for the Monarch machines listed above and located at Anniston Army Depot, Anniston, Alabama which includes but is not limited to:
A. Contractor shall make any and all adjustments to the machining centers to insure they meet the original alignment and tolerance requirements, and provide comparisons to the original “Inspection Report” recorded at machine installation.
B. The Contractor shall level work-surface and column ways, check and adjust parallelism between work-surface and column, check machine squareness, laser align all axes, check head tracking, check tool changer, check spindle unit offsets/sweeps, prepare machine inspection report, and make all required software/parameter adjustments. A copy of the software/parameter changes shall be provided to the government.
C. A machine inspection report shall be prepared containing all measurements taken and be provided to the Contracting Officers Representative (COR) immediately after the alignment has been completed.
D. Contractor shall provide recommendations, in the machine inspection report, for the Government to maintain the Machining Center within proper alignment and accuracy requirements.
E. Contractor shall provide all tools, labor, transportation, repair parts, documentation, maintenance supplies, test equipment, and any other related services, to inspect, align, and adjust the Machines.
F. If during the conduct of the annual inspection and alignment the contractor discovers that certain machine parts are in need of replacement the contractor shall furnish and install all such repair parts as a part of this inspection and alignment. Any such repair parts that are provided during such annual inspection and acceptance shall be separately identified and approved by the
Anniston Army Depot Contracting Officer prior to installation.
G. Any required expendable items shall be provided by the contractor as part of the
Annual Inspection and Alignment.
2.2 Repair/Emergency repair:
Emergency repairs shall be performed by Contractor personnel that are experienced and knowledgeable in all phases of repair, service, and operations of the machining centers.
Emergency repairs shall consist of any work that is necessary to restore the machine to a
100% performance level. The Contractor must be able to provide high priority repairs for the Machining Centers. Charges associated with emergency repairs including travel, labor, transportation, repair parts, mileage, etc., shall be invoiced to the Government. All prices shall be consistent with the Contractor's published prices that are in effect at the time of services. Only new OEM repair parts or components, or certified replacements of equal quality, shall be used to repair the equipment. If repair parts are not available on-site, contractor shall provide all repair parts and maintenance supplies and invoice separately to the Government. Any replacement part or parts shall be approved by the Contracting
Officer, Anniston Army Depot prior to installation. All replaced parts shall become property of the Government, unless there is a rebate available from the contractor for such parts.
3.0 Documentation of Malfunctions and Service Calls:
The Contractor shall furnish to the Government a service document which will be used to report the date and time of notification, along with a brief description of each malfunction reported to him. The Contractor shall furnish to the Government one copy of this report form upon completion of any corrective or preventive action taken by the Contractor. The report form shall include as a minimum the following:
A. Date and Time of Notification
B. Date and Time of Arrival
C. Time Spent for Repair
D. Description of Malfunction and/or Maintenance Performed
E. Description of Parts Replaced
4.0 Normal Period of Support (NPS):
The normal period of maintenance support shall be Monday through Thursday 0700-1630 hrs.
and 0700-1530 hrs. every other Friday. Maintenance support shall be made available to the Government during this period at a fixed hourly rate.
5.0 Support Outside Normal Period of Support (SONPS):
On-call emergency maintenance support shall be made available to the Government outside the Normal Period of Support (NPS) (including Federal holidays and weekends) at a fixed hourly rate (or rates), as specified in this contract.
6.0 Contractor’s Point of Contact:
The Contractor shall provide the Contracting Officer with a point of contact, email address and a telephone number for the Government to report service calls, and shall make arrangements for qualified service personnel (e.g. qualified technician) to receive notification and respond to the Government's request for technical support within twenty-four (24) hours after notification that services are needed. Contractor shall have a repair technician on site within seventy-two
(72) hours of initial notification. Telephone recording devices and/or voice-mail systems are acceptable. The Contracting Officer shall supply the Contractor with the name, email and telephone number of his/her designated technical representative. The COR is authorized to request maintenance support services. The Contractor shall not proceed with any verbal or written change(s) to the scope of work from sources other than the Contracting Officer, who will provide changes in writing.
7.0 Contractor Service Personnel:
The Contractor shall provide personnel that are experienced and knowledgeable of the machines at ANAD. Work shall be performed in accordance with the best commercial practices without unnecessary delays. Contractor personnel shall have the expertise to perform any and all repairs and adjustments for this equipment. 8.0 Response Remedies:
If, twenty-four (24) hours after the Contracting Officer or COR notifies the Contractor, the Contracting Officer determines that the contractor is not making continuous progress toward remedying any reported equipment malfunction, the Government shall reserve the right to secure the services of other commercial or Government qualified technicians to repair, replace, or otherwise correct the reported malfunction of the equipment at the Contractor’s expense.
9.0 Replacement Parts/Materials:
Replacement parts shall be supplied by the Contractor on a cost reimbursement basis as specified in this contract. Only new standard parts or parts refurbished equivalent to new, shall be used in effecting repairs. A discount on all parts and all upgrades shall be offered to the Government as part of this emergency on-call maintenance contract.
10.0 Warranty of Parts/Materials:
Parts/materials replaced under this contract will be subject to the standard commercial warranty applied to those parts or materials. Labor shall have at a minimum 30 days of warranty.
11.0 Surveillance/Communication:
All technical surveillance of on-call, on-site services will be performed by the Contracting Officer’s Representative. After correction of each malfunction, the Contractor shall notify the contract administrator named in this contract. Communication of all technical matters pertaining to this contract shall be in writing directly between the Contractor and the Contracting Officer, Anniston Army Depot, ATTN: AMSTA-AN-CTCS, 7 Frankford Avenue, Anniston, Alabama 36201-4199, with a copy furnished to the COR.
12.0 Contractor Responsibilities:
12.1 The Contractor shall provide all parts, tools, labor, documentation and materials necessary to perform the services required by the terms of this contract.
12.2 The offered shall include the following in his response to this request for proposals:
A. Hourly labor rate to be charged during the Service Outside Normal Period of Support (SONPS) for the base year and each one-year option.
B. Hourly labor rate to be charged during the Normal Period of Support (NPS) for the base year and each one-year option.
13.0 Contractor Manpower Reporting:
The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the [NAMED COMPONENT] via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address:
http://www.ecmra.mil/.
Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2013. 14.0 Access and General Protection/Security Policy and Procedures:
A. This standard language is for Contractor employees with an area of performance within an Army-controlled installation, facility, or area. The Contractor and all associated subcontractors employees shall provide all information required for background checks to meet installation access requirements to be accomplished by the installation Provost Marshal Office, Director of Emergency Services, or Security office. The Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA, and/or local policy. In addition to the changes otherwise authorized by the change clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in Contractor security matters or processes.
B. For Contractors that do not require a Common Access Card (CAC), but require access to a DOD facility or installation. Contractor and all associated subcontractor employees shall comply with adjudication standards and procedures using the National Crime Information Center interstate identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190-13), applicable installation, facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.
C. If necessary, Contractor shall acquire and return all issued U.S. Government Common Access Cards, installation badges, and/or access passes.
