Statement of Work Lockout / Tagout (LOTO) Procedure Writing Service
1.0 Scope: Anniston Army Depot (ANAD) requires Occupational Safety and Health Administration (OSHA) compliant Lockout/Tagout (LOTO) procedures at the point of use for 3,000 pieces of equipment and machinery. ANAD estimates 2,900 pieces of Type I equipment (1-4 energy sources) and 100 pieces of Type II equipment (5 or more energy sources). Each piece of equipment requires machine-specific graphical “LOTO Procedures” (hereafter Procedure(s)). ANAD equipment has equipment with energy sources that range from electrical, mechanical, hydraulic, pneumatic, chemical, gas, or thermal. Procedures are required to be compliant with OSHA’s standards for LOTO, which are contained in The Control of Hazardous Energy (29
CFR 1910.147).
2.0 Contractor shall:
2.1 Perform a comprehensive review for OSHA compliance on each piece of equipment in each building identified by the Government Contracting Officer’s Representative (COR).
2.2 Identify equipment requiring Procedures at ANAD.
2.3 Establish Procedures to meet 29 CFR 1910.147 for each piece of non-compliant equipment.
2.4 Follow the government provided template or approved equivalent. (See
Example 1)
2.5 Provide Procedure which shall:
2.5.1 Be physically attached to each machine, isolation point, and circuit breaker as required.
2.5.2 Be sized 8.5 x 11inches unless otherwise approved by the government.
2.5.3 Use multiple pages as required.
2.5.4 Be printed on bright white GP Card Stock or equivalent.
2.5.5 Contain photographs which are subject to government review and approval.
2.5.6 Contain a general photograph of the machine.
2.5.7 Contain photographs which clearly show LOTO isolation points.
2.5.8 Utilize photographs of high enough resolution to not pixelate.
2.5.9 Utilize graphics on the photographs to identify each LOTO location, such as arrows and circles.
2.5.10 Use non-fading inks or dyes.
2.5.11 Be laminated with Apache 5 Mil letter sized 8 x 11 inches, Crystal
Clear, 5 mil or equivalent, with UV protection.
2.5.12 Utilize a metal grommet to affix to equipment.
2.6 Provide Procedure which shall include:
2.6.1 At a minimum, the five (5) steps to apply LOTO. These steps are located on the template document (see example), and are required to be machine-specific. They include:
2.6.1.1 Step 1: Notify affected personnel
2.6.1.2 Step 2: Shut down the equipment, using normal stopping procedures for the machine.
2.6.1.3 Step 3: Isolate the equipment from the energy source.
2.6.1.4 Step 4: Apply LOTO devices, locks & tags.
2.6.1.5 Step 5: Verify isolation.
2.6.2 At a minimum, the five (5) steps to remove LOTO. These steps are located on the template document (see example), and are required to be machine-specific. They include:
2.6.2.1 Step 1: Ensure all items have been removed.
2.6.2.2 Step 2: Verify that all employees are safely positioned and that the controls are in neutral.
2.6.2.3 Step 3. Remove lockout devices
2.6.2.4 Step 4. Reenergize machine.
2.6.2.5 Step 5. Notify affected employees that machine has been returned to service.
2.6.3 Identify any circuit breaker box listed on the Procedure.
2.7 Have the following capabilities:
2.7.1 Be able to trace power sources from equipment to isolation without government assistance.
2.7.2 Utilize an Equipment Hazardous Energy Survey. Follow
government provided template or approved equivalent. (See Example 2)
2.7.3 Provide all resources and equipment required to identify isolation points.
2.7.4 Supply all equipment necessary to perform the scope of this contract.
2.7.5 Verify the effectiveness of each Procedure.
2.8 Install LOTO procedures on each machine in a readily accessible location.
2.9 Provide ANAD with a database of electronic files of completed LOTO procedures and Equipment Hazardous Energy Surveys. These files must be editable and compatible with Microsoft Office or Adobe software suites.
These files must include the following information:
2.9.1 Machine nomenclature/description
2.9.2 Unique Procedure Identification (ID)
2.9.3 Equipment ID
2.9.4 Cost Center
2.9.5 Location inside building
2.9.6 All photos. The contractor shall name photos in a way that relates to the specific equipment.
2.10 Provide ANAD with a final product that provides all the tools necessary to update and maintain the LOTO procedures in-house with no proprietary software or annual fees.
2.11 Provide information on OSHA compliant LOTO equipment for use on each procedure, such as:
2.11.1 Locks and tags
2.11.2 Hasps and lock boxes for group lockout
2.11.3 Specialized valve devices for non‐lockable valves
2.11.4 Plug devices to lock out electrical plug disconnects
2.11.5 Breaker devices
2.11.6 Fuse devices
2.11.7 LOTO tags to use with the locks
2.11.8 Specialized locks that are exclusively used for LOTO
2.12 Provide OSHA compliant training for an estimated 100 authorized employees on LOTO procedures as well as training materials for ANAD’s use and distribution. Contractor will also provide OSHA compliant training materials for affected employees. The following sub-bullets define authorized and affected employee training requirements as defined in the OSHA standard.
2.12.1 Authorized employees. These employees shall receive training on the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control [29 CFR 1910.147(c)(7)(i)(A)].
2.12.2 Affected employees. These employees must receive training on the purpose and use of the energy control procedure [29 CFR 1910.147(c)(7)(i)(B)].
3.0 Project Completion Requirements:
3.1 The period performance for this contract will be 6 months from contract award.
3.2 This contract shall have a performance period of 120 days after contract award to complete all requirements of this specification in CLIN 0001 and
CLIN 0002
3.3 CLIN 0003 shall be bid in increments of 100 for Type 1 Equipment and completed within the same timeframe as CLIN0001 and CLIN0002 unless a different time period is agreed to between the USG and contractor. The contractor will be notified in 10 working days or less in writing of additional work from the contracting office.
3.4 CLIN 0004 shall be bid in increments of 10 for Type 2 Equipment and completed within the same timeframe as CLIN0001 and CLIN0002 unless a different time period is agreed to between the USG and contractor. The contractor will be notified in 10 working days or less in writing of additional work from the contracting office.
3.5 Acceptance of work completed shall be done by the assigned COR.
3.6 All work shall be performed during the normal ANAD work schedule unless otherwise approved by the COR.
4.0 Government. ANAD will:
4.1 Respond to questions regarding this contract within one business day.
4.2 Coordinate the approval of all pictures used by the contractor.
4.3 Provide the contractor with a conference room consisting of 1 table and 8 chairs. Internet access will not be provided.
4.4 Reserve the right to exclude certain equipment from this contract.
5.0 Bid Schedule.
CLIN: 0001 2900 Pieces of Type I Equipment (1-4 energy sources)
CLIN: 0002 100 Pieces of Type II Equipment (5 or more energy sources)
CLIN: 0003
Option – Type 1 Equipment (1-4 energy sources). Priced in increments of 100.
CLIN: 0004
Option –Type 2 Equipment (5 or more energy sources) Priced in increments of 10.
