GENERAL INFORMATION
1. GENERAL: This is a non-personal service contract under which the personnel rendering the services are not subject, either by the contract’s terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the Government and its employees. The United States (U.S.) Government shall not exercise supervision or control over the Contractor’s employees performing the tasks herein. Such contract employees shall be accountable solely to the Contractor who, in turn shall be responsible to the U.S. Government.
Description of Services/Scope: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Peer
Intervention Training on Firearms and Lethal Means Safety as defined in this PWS.
Background: The military’s suicide prevention approach as one that is (though well-intended), over reliant on mental illness models, ‘warning signs,’ screening, and forms and paperwork. Conversely, a public health approach proposes that the most effective way to prevent illness, injury, and mortality involves strategies that focus on the hazardous condition itself. In the US Military that hazard is the firearm where suicide deaths involve firearms 67% of the time – grossly disproportionate from the civilian community. Suicide experts unequivocally agree that the one place where there is decided scientific agreement is putting time and distance between a person in crisis and the potential means of their suicide – this is lethal means reduction. Strategically, lethal means reduction is our most powerful suicide prevention approach, but its use is not widespread for several reasons. That the intended audience (including our leaders) is not buying it is a massive communication problem across our ranks. We must improve in this area by bringing the conversation about suicide risk and firearms into an area of trust – our Soldiers’ relationships with their peers and leaders. Research demonstrates that lethal means reduction is effective and translatable with military members when the right message is delivered by the right messenger. To that end, we must endeavor to achieve the support peer to peer intervention models that inspire action with their buddies in crisis, asking to temporarily hold onto their weapons or take protective storage measures –before it’s too late.
1.1 Objectives: The period of performance for this contract shall be for one (1) 12-month Base Period with four (4)
12 month Option Periods. Provide peer intervention training focused on firearms and lethal means safety.
Overarching goal of the training is a reduction in suicides and suicide ideations.
1.2 Scope: Contractor shall provide non-personal services in support of lethal means safety training.
1.3 General Information:
1.5.1 Quality Control Plan: The Contractor shall be responsible for quality control. The Contractor shall be responsible for the delivery of quality services/supplies to the Government under the terms of the contract and IAW
FAR 52.212-4, Contract Terms and Conditions – Commercial Items. The Contractor shall develop and maintain an effective Quality Control Plan (QCP) program to ensure services are performed IAW this PWS. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s QCP is the means by which the Contractor assures that Contractor work complies with the requirements of the contract. The Contractor’s QCP shall be structured to assure independent inspections of the Contractor’s work performance. The Contractor shall schedule and conduct QCP inspections. The QCP inspection schedule and the
QCP inspection results shall be available for Government review during normal business hours. Copies of any QCP documentation, either digital or paper, shall be provided within two (2) business days of KO request. The
Contractor shall deliver the quality control plan (QCP) within 10 business days of award of the first task order. The
QCP shall be applicable for all future task orders, unless the Contractor notifies and provides the KO of a change to the QCP. The Contractor shall receive the KO approval in writing, after acceptance of the QCP.
1.5.1.2 The Quality Control Plan shall address as a minimum:
1.5.1.2.1 Inspection Program: Included shall be a quality control inspection program covering all general and specific tasks included in the contract scope of work. It shall specify tasks or areas to be inspected on a scheduled or unscheduled basis, the manner in which inspections are to be conducted, the titles of the individuals who will perform the inspections, and the percentage of the work that will be inspected on a recurring basis. In developing this inspection program, the Contractor shall identify the key activities and associated characteristics in each process that have a significant influence on specific services and provide for methods for evaluation of the selected characteristics.
1.5.1.2.2 Deficiency Identification: The QCP shall include a method of identifying deficiencies in the quality of services performed before the level of performance becomes unacceptable as defined in the PRS. This shall contain processes for corrective action without dependence upon Government direction.
1.5.1.2.3 Deficiency Correction: The program shall contain process control and process performance measurement procedures that shall include how the Contractor shall implement preventive corrective actions. The Contractor shall demonstrate that it has designed quality into the delivery of services thereby mitigating the risk(s) associated with delivery of deficient or nonconforming services.
1.5.1.2.4 Documentation and Enforcement: The QCP shall include a method of documenting and enforcing quality control operations of both prime contractor and subcontractor work; including inspection and testing.
1.5.1.2.5 Trend Analysis: The QCP shall include a method of performing trend analysis and assessments through the use of metrics.
1.5.1.2.6 Surveillance Methods: The QCP shall contain specific surveillance techniques for all contract services.
The surveillance methods shall be comprehensive and adaptable to the reporting system of the plan.
1.5.1.3. Quality Assurance: The Government has the right to inspect all services called for by this contract to the extent practicable, at all times and places during the term of the contract. The Government will perform these inspections in a manner that will not unduly delay the work. The Government will inspect the Contractor’s performance in accordance with a Quality Assurance Surveillance Plan (QASP) that is tailored specifically to this contract. This plan is for the Government’s use, and is not provided to the Contractor for any purpose. The
Government, not the Contractor, performs Government quality assurance and surveillance actions to assess the extent to which the Contractor is meeting the performance quality requirements of the contract. The standards of performance of services performed under this contract are listed in the PRS located in the PWS. The lack of a QASP or Government surveillance does not in any way relieve the contractor of responsibility for meeting the contract quality requirements.
1.5.1.3.1 Non-Conforming Performance: All Government discovered non-conformances shall be documented at the time of discovery. Documentation will, at a minimum, include the contract and order number, reference to the specific contract requirement, the specific deficiency to the requirement, where it was discovered, the date and time it was discovered, focal point for the contractor who was notified and suspense date for contractor response. The
COR will notify the Contractor, in person, each time a non-conformance is observed. The Contractor will be given a time line after notification to correct the deficiency IAW requirements of the contract. Discrepancies that cannot be corrected within the identified time line will be reported to the KO. Non-conformance will be tallied for Contractor
Performance Assessment Rating (CPAR) reporting periods and the results of the data will be used to substantiate objective ratings of contractor performance.
1.5.1.3.2 Non-Conformances: Non-conformances shall be classified in one of three categories: Level I (Minor), Level II (Major) or Level III (Critical):
Level I (Minor) nonconformance is defined as a nonconformance that is not likely to materially reduce the usability of the supplies or services for their intended purpose, or is a departure from established standards having little bearing on the effective use or operation of the supplies or services.
Level II (Major) nonconformance is defined as a nonconformance, other than critical, that is likely to result in failure of the supplies or services, or to materially reduce the usability of the supplies or services for their intended purpose. The Contractor shall provide a Corrective Action Plan (CAP).
