Amendment 0001 – W90QV7-24-Q-A008 Hoods & Ducts

1. First, in the case of B510, the CLIN price input methods for Solicitation and PWS are different.

Which method should I enter the price for? See below.

In Solicitation, B510 is supposed to be priced every month like the other buildings, but in PWS, B510 is supposed to be priced every quarter.

5.2.1. The contractor shall adhere to the cleaning frequencies outlined in TE#1 (Monthly/Quarterly). All facilities and hood and duct systems that require only quarterly cleaning are specified in TE#2. Additionally, all hood and duct systems that are used specifically for deep frying will be completed monthly IAW TE#2. The contractor shall not be deviate from these cleaning frequencies without prior approval of the KO.

For CLINs that require quarterly cleaning, you will multiply the price of one cleaning times 2 to get the total price. Then you will divide the total price by 6 (Base Year) as follows:

For example, W100,000 x 2 = W200,000. W200,000/ 6 = W33,333 for the Base Year Unit Price.

For the option years, you multiply the price of one cleaning times 4 to get the total price. Then you divide the total price by 12 as follows: W100,000 x 4 = W400,000. W400,000/ 12 = W33,333 for the Option Years Unit price.

2. The solicitation does not include a request to submit a Tax Exemption Declaration document, but you posted it as an attached file on the SAM site. Do I have to fill out the Tax Exemption Declaration document and submit it together to bid for this hood & duct cleaning?

Yes

3. You didn't upload the quotation file that enters the price separately on the SAM site, so can I submit the price by entering it into solicitation and PWS?

You can input the price in the solicitation, PWS, or in any format you like.

4. If you look at page 21 of solicitation, the field for entering Address and POC is blank. I would like to know whether we should enter our information here or leave it blank. Leave Blank.

5. On the first page of Solicitation, the proposer is asked to enter blocks 12, 17, 23, 24, AND 30, but I am not sure what to enter in block 12, so please let me know. Leave Blank.

6. When do we clean the Hoods & Ducts?

5.2.2. The contractor shall provide the COR with a monthly work schedule no later than the twentieth (20th) day of the month prior to the scheduled services. This schedule shall describe what type of service is being performed; kitchen hood and duct cleaning, the facility number, the desired day of service, and morning or afternoon (A.M. or P.M.) cleaning.
