CLAUSES INCORPORATED BY FULL TEXT
52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
Factor A: Technical
Sub-factor 1. Resume: Resume must show evidence of at least two (2) years full-time experience in care coordination, discharge planning or case management in a medical (military or civilian) setting within the last four
(4) years.
Sub-factor 2. Education: A transcript (official or unofficial) that reflects the following: Nurse applicants must possess a baccalaureate degree program in nursing (BSN) accredited by a national nursing accrediting agency recognized by the US Department of Education. Social Worker applicants must possess a Masters of Social Work (M.S.W.) degree from a graduate school of social work fully accredited by the Council on Social Work Education
(CSWE).
Sub-factor 3. License: The license shall reflect the following: Nurse applicants must possess a valid, unrestricted license to practice as a Registered Nurse (RN) from one of the 50 states, and/or the District of Columbia, the Commonwealth of Puerto Rico, or territories of the United States. The contractor must be in good standing, and under no restrictions, with the state in which a license is held or has been held, or the national licensure board, within the last 10 years. Social Worker applicants are not required to possess a license to apply.
Sub-factor 4. Current Certificate: The current certificate shall reflect the following: American Heart Association Basic Life Support (BLS) (Course C) or the American Red Cross CPR/BLS Course.
Acceptable Proposal clearly meets the minimum requirements of the solicitation
Unacceptable Proposal does not clearly meet the minimum requirements of the solicitation
Factor B: Price: The offeror shall provide complete CLINs identified for base period plus four (4) options.
Proposed price will be reviewed to determine if the proposal is adequate, complete, and reasonable to assess the offeror understands of the solicitation.
As part of price evaluation, the Government will evaluate its option to extend services (FAR Clause 52.217-8 - Option to Extend Services) by adding one-half of the Offeror’s final option period price to the Offeror’s total price.
Factor C: Past Performance: The past performance evaluation is an assessment of the offeror’s probability of meeting the minimum past performance solicitation requirements. This assessment is based on the offeror’s record of relevant and recent past performance information that pertain to services outlined in the solicitation. The offeror shall provide a list of contracts under which they have performed the same or similar services as to this RFQ within the last four (4) years from the issuance date of this solicitation. This data on recent contracts will be used by the Contracting Officer to evaluate past performance.
The following information shall be included as applicable:
• The title of the Project or Contract No for the Commercial or Government performance
• A clear description of qualifying work experience
• Length of contract/project (Year/Month/Date – Year/Month/Date)
• Value of contract/project total amount
• Complete POC of employer (including name, phone no, e-mail) and validating
POC.
The following ratings will be used in evaluating Past Performance:
Acceptable Based on the Offeror’s performance record, the Government has a reasonable expectation that the Offeror will successfully perform the required effort, or the Offeror’s performance record is unknown (See note below)
Note: In the case of an Offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the Offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)).
Therefore, the Offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.
(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
(End of provision)
