In an effort to assist the USG in assessing your performance risk, offerors must have each of their subcontractors and teaming partners complete Attachment 3 entitled “Subcontractor/Teaming Partner Consent letter”. The word “contract” is defined as a single contract, purchase order, or a TO on an Indefinite Delivery/Indefinite Quantity (IDIQ) contract. The word “contract”, as defined herein, does not mean basic IDIQ contract, for the purpose of past performance submissions under this solicitation. A single performance assessment submission shall only include one (1) contract, TO or purchase order and MAY not include a combination of contracts, purchase orders, or TOs. If an offeror submits more than three contracts, the USG will evaluate only the first three listed contracts for recency and relevancy. If an offeror submits multiple contracts, purchase orders or TOs on one (1) performance assessment submission, then that submission will not be evaluated. Performance assessments shall meet the relevancy definition in Addendum to 52.212-2 and performance must have occurred within the last three years. The total number of performance assessment submissions shall include prime and any major subcontractor(s) projected to perform on any resultant contract. Only those subcontractors that meet the definition of a major subcontractor, as defined as performing more than 25% of any resultant contract arising from this solicitation will be used in the offeror’s performance assessment. PRAQs should be completed by a member of the program office, administrative contracting office, end users of the service provided, or any other USG technical or business advisor who has knowledge of the Contractor’s performance. The USG will use the results of submitted questionnaires as a method to evaluate an offeror’s past performance. The offeror shall provide a copy of the attached PRAQs to each POC with a letter the same as or similar to that at Attachment 6 entitled “Sample Questionnaire Cover Letter”. Instruct each source to complete and e-mail the entire questionnaire to Mr. Joe E. Watson at e-mail: joe.e.watson2.civ@mail.mil. Upon examination of the initial proposals, the contracting officer will review this determination and if in the Contracting Officer’s opinion, adequate competition exists, no additional cost information will be requested. Additionally, all information related to pricing shall be included in Volume IV of the proposal only. UNDER NO CIRCUMSTANCES should pricing information be included elsewhere in the proposal. A complete Volume IV shall include the information below:
1. Volume IV shall include pricing information that will be used to evaluate an offeror’s total price. The offeror shall include pricing for each Contract Line Items Number (CLIN) on the SF1449 Continuation Sheet for the base and all options; except Contractor Manpower Reporting CLINs. The offeror shall only provide information essential to document or support the offeror’s price proposal.
2. CONTRACTOR MANPOWER REPORTING (CMR). Offerors shall not provide separate pricing for annual CMR requirements.
3. PRICING NARRATIVE. The price proposal shall a include narrative as to how the prices were developed for each of the CLINs. Please show all of the cost elements that make up each unit price for each CLIN and include the methodology used to determine the number of personnel you will utilize to execute the required services.
8. VOLUME V - SMALL BUSINESS PARTICIPATION PROPOSAL
Small Business Participation Proposal (Format Example)
All offerors (both large and small businesses) are required to complete a Small Business Participation Proposal. Offerors should propose the level of participation of small businesses (as a small business prime, joint venture, teaming arrangement, and/or small business subcontractors) in the performance of the acquisition relative to the objectives/goals set forth in the evaluation of this area.
(a) Check the applicable size and categories for the PRIME Offeror only -- Check all applicable boxes:
{ } Large Prime or
{ } Small Business Prime; also categorized as a { } Small Disadvantaged Business (SDB) { } Woman-Owned Small Business (WOSB) { } Historically Underutilized Zone (HUB Zone) Small Business { } Veteran Owned Small Business (VOSB) { } Service Disabled Veteran Owned Small Business (SDVOSB)
(b) Submit the total combined dollar value and percentage of work to be performed by both large and small businesses (include the percentage of work to be performed both by Prime, joint venture, teaming arrangement, and subcontractors):
Example: If the Prime proposes a price of $1,000,000 (including all options), and small business(es) will provide $250,000 in services/supplies as a prime, joint venture, teaming arrangement, or subcontractor, the % planned for small businesses is 25%; and 75% for large business, equaling 100%.
Total Percentage planned for Large Business(es) ____75_% = $ 750,000
Percentage planned for Small Business(es) ____25_% = $ 250,000
100% = $1,000,000
When combined, Large and Small Business totals must equal 100%.
(c) Please indicate the total percentage and dollar value of participation to be performed by each type of subcategory small business. The percentage of work performed by Small Businesses that qualify in multiple small business categories may be counted in each category:
Example: Victory Prop Mgt (WOSB and SDVOSB) performing 2%; and Williams Group (SDB, HubZ and WOSB) performing 3%. Results equate to: SB 5%; SDB 3%; HubZone 3%; WOSB 5%; SDVOSB 2%; VOSB 2%;). SDVOSBs are also VOSBs automatically; however VOSBs are not automatically SDVOSBs.
| Small Disadvantaged Business | _________% $_________ | |
| HUB Zone Small Business | _________% $_________ | |
| Woman Owned Small Business | _________% $_________ | |
| Service Disabled Veteran Owned SB | _________% $_________ | |
| Veteran Owned Small Business | _________% $_________ |
(d) Identify the Prime Offeror and type of service/supply that the Prime Offeror will provide. Then list each of the intended subcontractors and principal supplies/services to be provided by that subcontractor. Provide the Commercial and Government Entity (CAGE) code for the Prime and each intended subcontractor. Also, provide the anticipated NAICS codes(s) that the Prime Offeror believes best describes the product or services being acquired by its subcontracts with each intended subcontractor. Small business Primes and small business subcontractors that qualify as small businesses in multiple small business categories should be listed in each applicable small business category.
Example: If a Small Business qualifies as a WOSB and a SDVOSB, you can add them to each category below in which they qualify.
(Include CAGE Code) for Each Subcontractor (1)
| Name of Company |
| Anticipated NAICS Code |
| Type of Service/Service |
Prime Offeror:
Large Business:
Small Business:
SDB:
WOSB:
HUBZone Small:
VOSB:
SDVOSB:
Notes:
(1) Pursuant to Sections 8(d) of the Small Business Act, a business is considered small for Government procurements if it does not exceed the size standard for the NAICS code that the prime contractor believes best describes the product or services being acquired. In other words, the size of the prime’s suppliers is determined by the applicable NAICS code of their joint venture, teaming partner, or subcontract, which may or may not be the same NAICS code as the one for your prime contract with the Government.
(e) Describe the extent of commitment to use small businesses (e.g., what types of commitments, if any, are in place for this specific acquisition either – small business prime, written contract, verbal, enforceable, non-enforceable, joint venturing, mentor-protégé, etc.) Provide documentation regarding commitments to small business for this effort. Copies of such agreements should be provided as part of your small business participation plan and will not count against the page limitation for this volume.
9. VOLUME VI – SMALL BUSINESS SUBCONTRACTING PLAN
It is the intent of the Government to ensure that small business concerns have the maximum practicable opportunity to participate in the performance of this requirement.
1. If the offeror is other than a small business, the offeror shall submit an Individual, Master, or Commercial Subcontracting Plan. The offerors plan must detail a logical and realistic approach in their establishment, oversight, and quality control of subcontracts which comply with FAR 19.702(a)(1) and (2) (Small Business Programs).
1. Current DOD goals are:
Small Business (32% of total contract value) Small Disadvantaged Business (5% of total subcontracting dollars) Woman-Owned Business (5% of total subcontracting dollars) Service Disabled Veteran-Owned Small Business (3% of total subcontracting dollars) HUBZone Small Business (3% of total subcontracting dollars)
1. Small Business Offerors: Each small business offeror shall list and describe any subcontracts it may use to perform the requirements of the PWS. If no subcontracting is projected, the small business shall state so in its proposal.
The approved subcontracting plan will be incorporated into the contract as compliance documents serving as the basis of the offeror’s subcontract management practices.
