A1. New contract

Q2. Can you provide a reflected ceiling diagram and/or a diagram/schematic with the layout on the rooms for each location?

A2. Not available.

Q2a. How many rooms, # of room-side HEPA filters, SQ FT, BSCs, etc?

A2a. 3 rooms – Anteroom, Positive Cleanroom, Negative Cleanroom

A2b. HEPA filters: Anteroom 2 filters, Positive Cleanroom 1 filter, Negative cleanroom 1 filter

A2c. Anteroom 115 sq. ft / Positive Cleanroom 59 sq. ft / Negative Cleanroom 68 sq. ft

A2d. BSC/Glove box locations: one in each cleanroom, centered on wall opposite the doors

Q3. Can you provide a copy of the previous report(s)?

- Viable sample plan (per previous locations)

A3a. See Attachment # 1 Ventilation Survey Results (Semi-Annual Ventilation Survey Results, Portable Laboratory Hood, 0687-HOSY-727A)

A3b. Not available.

Q4. What system is in place to perform the test/introduce challenge (picture of the filters/challenge port)?

A4. Airflows

Purpose: The purpose of this test is to determine the air velocity, and uniformity through the HEPA/ULPA filters and/or work zone. To determine supply airflow capacity through each ceiling filter.

Procedure: Using a barometer with digital micro anemometer, seal hood over each supply vent and filter to measure captured airflow volume, or divide the filter face area or work zone area into 1’ x 1’ square grids and take velocity readings in the center of each grid. Record all individual airflow readings and average for uniformity and total air change calculation.

Q5. What is the current certification schedule (which months)? When is the next certification due?

Q5a. Q5b. Q5c.
