About this file

This document contains a performance work statement (PWS) for repair and maintenance of government-owned surgical instruments at Tripler Army Medical Center in Hawaii. The PWS requires on-site and off-site repair and preventative maintenance services for approximately 40,400 surgical instruments per year. Services include sharpening, adjusting, cleaning, realignment, and replacement of worn, broken, or lost parts to restore instruments to like-new condition. The contractor must respond within 24 hours for on-site and 48 hours for off-site repairs, and correct any deficiencies within five days of notification. The contractor will be evaluated quarterly on maintaining 98% uptime of all equipment. The PWS specifies response times, licensing requirements, quality control measures, and safety and security procedures the contractor must follow while working on-site at the medical center.

SECTION SF 30 BLOCK 14 CONTINUATION PAGE

The following items are applicable to this modification:

A. The purpose of this amendment is to provide Questions & Answers as follows:

(1) Question: I do not see the list of surgical instruments on the solicitation.

Answer: The instruments are listed on pages 53 – 54 of the solicitation. A current inventory list is provided at Appendix A.

(2) Question: The service schedule says preventive maintenance will be performed on a weekly basis at a minimum of three times a month. What is the anticipated length of the weekly visit? Five days? One day?

Answer: The length of each visit is usually one day, unless there are some sets that were not completed/requires further maintenance.

(3) Question: The off-site service. Our technicians can repair 12-15 trays per day. They are also well equipped for on-site repairs. Are there any particular instruments or devices that you anticipate needing off-site repair?

Answer: Some instruments that requires off-site services are micro instruments (especially eye sets) and insulated bipolar. But if the technician can perform the job on-site, there is no need for off-site repair.

(4) Question: The PWS lists 42 trays on pages 53 and 54. I do not see how this translates to 30,000 instruments per year. Are the trays to be rotated a specific number of times a year? Are there additional Instruments not included in the list?

Answer: Although it says 42 trays on pages 53 and 54, there are multiple numbers for each. Our inventory changes all the time, as stated in PWS (scope of work), “3.1. Equipment to be serviced includes, but not limited to..”.

(5) Question: Is there any way we can get an inventory list of the surgical instruments?

Answer: Current inventory is provided as Appendix A.

(6) Question: Are there currently personnel on site? If so how many?

Answer: Technically there are no personnel on-site. Due to limited space, TAMC does not/will not provide workspace for the contractor to perform “on-site” repairs. Contractors are responsible to provide their own workspace to perform “on-site” repairs. See paragraph 3.4.1 of the PWS.

(7) Question: Box 23 and 24, UNIT PRICE and AMOUNT – Because the amount of repairs per visit is not a fixed number, should we respond with the fixed daily price per visit as is the experience of this vendor?

Answer: Prices shall be stated for each CLIN beginning on page 3 through page 11. Unit Prices shall be stated as cost of repair per item and Amount is the Quantity multiplied by the Unit Price (i.e. 30,000 x $X(unit price) = $Amount). The solicitation is not structured for unit pricing as fixed daily price per visit. The contractor will only be paid for repair services actually provided and contract amount will be adjusted up or down, as necessary to reflect the value of the actual repair services provided. See paragraph 3.7.6 of the PWS. For example, if in one month you repair 2,000 instruments, then you will invoice the Government 2,000 x $unit price = amount for that month invoice.

(8) Question: Pages 3 – 12 of 96 – NET AMOUNT – It is my experience that the price is per/visit. For example $XXXX per day per visit is what I previously quoted for specialty repair. I’m not sure how to put that into a monthly or yearly metric as quantity and volume fluctuate unless price is to reflect the 3 visit per month schedule. I know that currently repairs are done twice a week. Please advise. In Net Amount I would be inclined to put a per/day price in net amount but I’m not sure if that’s acceptable.

Answer: See answer above in Question (7). The solicitation is not structured for unit pricing on a per/day basis, unit prices are to be “EACH” cost of repair per item. The Net Amount is the same as the total AMOUNT for each CLIN. The AMOUNT total is carried down to NET AMOUNT.

B. Clarification is provided for the provision at DFARS 252.232-7998 Supplier Self-Service Pilot (Deviation

2018-A0001)(Dec 2017), page 79 of the solicitation, as instructed in the provision paragraph (2) “Offerors shall indicate in their offer whether they want to be considered for participation in the pilot”. Your response is required, as stated on page 64 of the solicitation under Volume I – Administrative. It is recommended you indicate your response within your submitted Volume I. A statement indicating whether you would or would not like to participate in the SUS Pilot is required. A negative response is required.

C. All further questions must be submitted in writing and be received no later than 23 October 2020, 08:00 am

HST.

D. As stated in Block 11 of the SF30, acknowledgment of this amendment must be received prior to the due date of quotes. See Block 11 for acknowledgement instructions.
