About this file
This is Standard Form 30 Amendment No. 0002 to solicitation W81K026QA064, issued by the W40M Medical Readiness Center Office West located at JBSA Fort Sam Houston, Texas, with an effective date of February 4, 2026. The amendment is administered by the same office and includes contact information for Sheryl Egans. The contracting officer is Timothy Hoerz, who signed the amendment on February 5, 2026.
The purpose of this amendment is to post Questions and Answers clarifying the prescription and authorization process for compound pharmaceuticals under this contract. The answer addresses that compound pharmaceuticals are designated for inpatient pharmacy settings and emergency departments as critical items (crash carts), which do not require individual patient prescriptions like outpatient pharmacy services. Instead, inpatient care and emergency critical responses operate under medication orders and authorizations from providers, which are billed as inpatient medical expenses. The answer notes that this practice is consistent with procedures at other Military Treatment Facilities (MTFs) and has been utilized at CRDAMC for an extended period. No other questions were received during this amendment cycle.

SUMMARY OF CHANGES
Purpose of this amendment is to post Block 14 (Description of Amendment/Modification):
Questions and Answers:
QUESTION 1: From a pharmacy standpoint, we normally can only dispense compounded drugs with a prescription. For government medical facilities, can you please explain how the prescription process works for this contract? Specifically, how are the drugs authorized and dispensed on the government side if the contractor is supplying product without receiving individual patient prescriptions?
ANSWER: First, the compound pharmaceuticals are designated to be used at Inpatient Pharmacy settings and/or emergency department as critical items (crash cart). Unlike outpatient pharmacy that dispensed compounded drugs require prescriptions, Inpatient care and emergency and critical responses do not require prescriptions. Instead, we either receive medication orders and authorizations from providers, and these are billed as inpatient medical expense.
Secondly, using compound pharmaceuticals for inpatient settings/emergency department with providers' orders are practiced among other MTF's as well. In addition, here at CRDAMC we have been doing this for quite some time that we utilize compound pharmaceuticals to provide Inpatient care and critical emergency response.
NO OTHER QUESTIONS RECEIVED.
