RFIS AND GOV RESPONSES

Government’s response to Vendors’ Comments, RFIs, and Recommendations

26 AUG 21

Vendor Comments Government Response Added to Salient Characteristic s

Gov Response

Anesthesia machines manufactured in [Foreign Country Redacted] and do not meet the Trade Agreement Act (TAA) compliant requirements for government acquisitions.

DFARS 252.225-7036 is incorporated in the Solicitation;

the clause specifies the definition of a Commercial-off-the-shelf (COTS) item, and states that COTS items are also defined as an “Qualifying country end product”

NO

Vendor Requests for Information

RFI 1 We recommend that the touch screen and touch responsive alarm system be removed. Many lower price point machines use hard keys and trim knobs to navigate around the menu.

Assessed - The Government requires a unit with Touch Screen

NO

RFI 2 Flow meter with dual tubes 0-1L/min, 1-10L/Min:

Many machines no longer use physical flow tubes but use digital representations of flow. This eliminates most anesthesia machines without any clinical value.

If the digital representations can be done at the 0- 1L/min and the 1-10Lmin range that would be acceptable.

YES

RFI 3 Ventilator drive gas auto switch that provides ventilation without interruption is overly restrictive.

Many machines no longer need a ventilator switch to start ventilation. The ventilator starts from the menu.

In addition, [Redacted] machines do not need drive gas for ventilation which provides more precise ventilation and saves on oxygen.

A system that has a built-in ventilator that does not require a drive gas would be acceptable.

The ventilator will need to be easily toggled on and off when required.

YES

RFI 4 Heated patient breathing circuit: Some machines heat the breathing system or heat the flow sensors but typically do not heat the circuit.

We require a warmed circuit in order to avoid heat loss and making the patient cold; no alternatives will suffice

NO

RFI 5 Inspiration and expiration flow sensors: This would eliminate some lower price point machines that have an expiratory flow sensor but monitor inspiratory flow sensing by a different mechanism.

Inspiration and expiration flow sensors are required.

NO

RFI 6 Oxygen sensor cable with Medical MOX-2 sensor:

The MOX-2 Oxygen sensor is specific to [Manufacturer Redacted] and only [Manufacturer Redacted] can provide it. Other machines can provide a galvanic O2 sensors but there are other O2 sensors that are non-consumptive such as that is used in the [Vendor Equipment Redacted]. Specifying the type of O2 sensor unless there is a definite advantage such as non-consumptive one is unnecessarily restrictive and eliminates some viable anesthesia machine options.

Other non-consumptive O2 sensors would be acceptable.

YES

RFI 7 Active Gas Scavenging System with activated charcoal canister bracket: We know of no anesthesia machine that provides a charcoal canister bracket without the activated charcoal device. An active gas scavenging system does not require activated charcoal. [Vendor Redacted] has an activated charcoal canister for a passive scavenging system for removal of the anesthetic agents but the activated charcoal does not remove the nitrous oxide gas and should not be used with nitrous oxide (N2O).

A passive scavenging system with an activated charcoal bracket for holding activated charcoal canisters is required.

YES

RFI 8 Mounting bracket for patient monitoring devices:

[Vendor Redacted] can provide the physiologic monitor but none is specified. We would need the model of the existing monitors to be mounted to the machine to quote the correct mount for the monitor/s.

Smith's Medical Surgivet Advisor

YES

RFI 9 Ohmeda-Style Oxygen Quick Connect: The assumption is that hoses will be provided. Please confirm.

Hoses need to be provided so we can connect to our existing O2 lines. CLINs structure was updated as a result.

YES

RFI 10 There is no requirement for suction apparatus. This is frequently quoted on anesthesia machines. Please confirm that this is needed.

No suction apparatus is required.

We have a standalone unit.

YES

RFI 11 When mounting other devices on the machine such as monitors and syringe pumps, a medical grade power outlet strip is frequently required. Please confirm this is a requirement.

This is not required. NO

RFI 12 Most anesthesia machines have optional accessory lighting. Please confirm that this is desired.

This is not required. NO

With the abovementioned restrictive salient characteristics in mind, we would like to propose the following minimum salient characteristics for your consideration:

GENERAL DESIGN

1 The anesthesia machine must be approved by the Federal Food and Drug Administration (FDA).

There is no requirement for Animal anesthesia units to be FDA Approved.

YES

2 The anesthesia machine must be suitable for general and regional anesthesia cases for adult, pediatric and neonatal patients without having to change any components of the device (only the breathing circuit).

Not Required; Unit shall meet industry standard requirements for animal use.

NO

3 The anesthesia machine must accept O2, N2O, and Air supplied by a medical gas pipeline system or by externally mounted gas cylinders.

Yes YES

4 The anesthesia machine must include gas hoses (at least 15-foot) and adapters for oxygen (O2), Nitrous Oxide (N2O), Medical Air, Vacuum, Waste Anesthetic Gas Disposal (WAGD) and E-cylinder Diameter Index Safety System (DISS).

Yes YES

5 The anesthesia machine will have a minimum of two storage drawers.

Yes YES

6 In the event of a mains power failure, the battery backup will support operation for a minimum of 30 minutes.

RA states that will purchase a separate Battery backup system to give them the flexibility to run multiple devices in the case of a power outage.

NO

7 The anesthesia machine will have a central brake. This is not required. NO

PRE-USE CHECKS

8 The anesthesia machine must include either a fully automated self-test with no user interaction required or semi-automated self-tests with limited user interaction required.

Yes YES

9 The anesthesia machine must have the ability to detect malfunctions of the ventilation delivery system such as leaks in the breathing circuit.

Yes YES

10 An abbreviated check will be available when the anesthesia machine is in Standby to permit testing of components that are changed between cases (e.g., checking the breathing circuit for leakage and compliance).

Not Required NO

Anesthetic agent delivery

11 The anesthesia machine must be suitable for volatile anesthesia delivery in a rebreathing system.

Yes YES

12 The anesthesia machine will have a safety mechanism that prevents dosing more than one agent at a time.

Already listed in salient characteristics.

NO

18 It must be impossible to deactivate the Oxygen (FiO2) alarms.

Not Required NO

Ventilation

19 The anesthesia machine will make possible manual and automatic ventilation as well as spontaneous breathing with or without pressure support.

Already included in salient characteristics

NO

20 During pressure supported ventilation a minimum back up rate must be adjustable for assuring minimum minute ventilation.

Yes YES

21 The anesthesia machine will be capable of delivering a tidal volume up to at least 1,400 mL to the patient.

Yes YES

22 The unit shall be able to provide both volume-controlled ventilation (with compliance compensation) and pressure-controlled ventilation.

Already listed in salient characteristics.

YES

23 The anesthesia machine will allow for the delivery of at least O2 and anesthetic agent without any power to the anesthesia machine for manual ventilation in emergency instances where care must be provided or continued immediately.

Not Required NO

Usability

24 The anesthesia machine will have a display rotary knob to facilitate fast, accurate adjustments to parameters and alarm settings.

Not required - See Government's response to RFI 1. Thought there may be a limited amount of rotary knobs/buttons, the anesthesia machine shall be primarily touch screen operated.

The accuracy of adjustments, etc. are implied.

NO

25 The anesthesia machine will offer a user interface that is consistent across the vendor's currently sold anesthesia machine portfolio.

Yes YES

Alarms

26 The alarm structure on the anesthesia machine will be hierarchical with each alarm falling into one of three categories: high priority, medium priority, and low priority. Each priority level must have a distinct audible and visual indication.

Not Required NO
