52.204-26 Covered Telecommunications Equipment or Services-Representation.
As prescribed in 4.2105(c), insert the following provision:
Covered Telecommunications Equipment or Services-Representation (Dec 2019)
(a) Definitions. As used in this provision, “covered telecommunications equipment or services” has the meaning provided in the clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.
(b) Procedures. The Offeror shall review the list of excluded parties in the System for Award Management (SAM) (https://www.sam.gov) for entities excluded from receiving federal awards for “covered telecommunications equipment or services”.
(c) Representation. The Offeror represents that it does, does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument.
The following have been modified:
ADDENDUM TO 52.212-1
The selection of a source for award purposes will be conducted utilizing Tradeoff source selection process. This process permits tradeoffs among cost or price and non-cost factors and allows the Government to accept other than the lowest priced proposal. Offers will be evaluated using the criteria under “Factors to be evaluated.”
Noncompliance with the Request for Quote (RFQ) may hamper the Government's ability to properly evaluate and may result in elimination of the quote from further consideration for contract award.
Award shall be made to the Tradeoff source selection process
(i) Technical: Vendor shall provide documentation that confirms their offer meets all salient characteristics in accordance with addendum to FAR 52.212-2.
Vendors interested in submitting an offer must furnish descriptive literature (brochures, specifications, dimensions, etc...) that confirms your offer meets the salient characteristics of the customer. Inability to do so will result in your quote being disqualified.
(ii) Delivery: Vendor shall submit their proposed lead-time within their offer.
(iii) Pricing: Offerors shall provide pricing information for CLINS (under 0001 - 0005) in their Entirety on the attached document labeled “PRICING SCHEDULE.” Unbalanced pricing may be evaluated in accordance with FAR 15.404-1(g), as applicable, to assess potential risk which could result in unreasonably high prices. This evacuation factor will not be scored or rated.
Submitting your quote: Vendors shall submit their company’s quote electronically to the email address specified in the Contract Opportunities (FBO) section of System for Award Management BETA https://beta.sam.gov of this Request for Quote (RFQ). Vendor may submit no more than one quote.
When submitting your offer via email please annotate the subject line as “Quote for W81K00-20-Q-0161” to joshua.p.barbero.civ@mail.mil no later than 11 August 2020 at 04:30 PM CST.
NOTE: LATE SUBMISSIONS - Bidders are responsible for submitting bids, so as to reach the Government office designated in the RFQ by the time specified in the RFQ. Any quote received after the exact date and time specified for receipt of quotes is “late” and may not be considered for award. The Contracting Officer has determined that accepting “late” quotes would unduly (excessively) delay this acquisition and procurement process from being awarded in a timely manner.
Requests for Information (RFI). All RFIs must be submitted no later than close of business 4 August 2020 @ 4:30 PM Central Standard Time (CST). Questions submitted after the specified date/time will not be accepted by the Government. Multiple emails for RFI’s will not be accepted. The Government reserves the right to respond to questions at a later date in order to conduct research and provide a response. Government responses and revisions to technical exhibits, Performance Work Statement (PWS), and any other supporting requirement documents will be incorporated into an amendment to the solicitation and posted to System for Award Management BETA https://beta.sam.gov website for viewing by all potential Offerors.
In accordance with FAR 52.212-3 ALT I , The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (v) of this provision.
Inability to register your representation and certifications via SAM or provide manual representation & certification (see FAR 52.252-1) will result in your quote being disqualified.
(m) The non-FAR Part 12 discretionary FAR, DFARS, AFARS, and LOCAL provisions included herein are incorporated into this solicitation either by reference or in full text. If incorporated by reference, see provision 52.252-1 herein for locations where full text can be found.
NOTE: Section C of Exhibit #1 FAR 52.204-26 must be returned or verify 52.204-26 has been completed with online Reps and Certs to be eligible for award.
