The following items are applicable to this modification:

AMENDMENT 01

The purpose of this Amendment 01 is as follows:

1. To revise the closing date from April 25, 2016 at 3:00 PM, Central Time to May 9, 2016 at 4:00 PM, Central Time to allow 30 days for solicitation to be posted to the web.

2. To add CLIN 0004 for the Guaranteed Minimum, Pre-Production Sample, which will be added to the weighted evaluated price to determine the Total Evaluated Price.

3. Revise Price Matrix, Attachment 0001 to include the Guaranteed Minimum Preproduction Sample.

**Please be sure to complete all Blue Cells in Pricing Matrix**

4. To revise Section M EVALUATION FOR AWARD, Paragraph 3., as follows:

1. The Government intends to award two or more five (5) year, Firm Fixed Price (FFP), Indefinite Delivery/Indefinite Quantity (IDIQ) contracts to the responsible offerors whose proposals are in compliance and adhere to all the terms and conditions of the solicitation with the Lowest Total Evaluated Price. The Government reserves the right to make a single award if determined to be in the Government's best interest.

2. The Government will evaluate offers based on prices proposed for all quantity ranges for all ordering periods. If an offeror fails to propose for all quantities or ordering periods, the Government may reject that offer as unacceptable.

3. For each proposal, the Government will calculate a weighted evaluated price by multiplying the proposed unit prices for each range by their respective weight and maximum quantity for all ordering periods, and adding the price of the Guaranteed Minimum (Pre Production Sample) to determine the total evaluated price. The weight associated with each range represents the likelihood that an order, if placed, would be within that range.

4. As part of the evaluation, proposals may be reviewed to identify any significant unbalanced pricing found between quantity ranges. In accordance with FAT 15.404-1(g), i.e., Unbalanced Pricing, a proposal may be rejected if the contracting officer determines the lack of balance poses an unacceptable risk to the Government.

5. Subsequent Delivery Orders will be competed in accordance with Fair Opportunities to compete provisions in FAR Part 16.505. The U.S. Government intends to compete delivery orders based on price. However, the U.S.

Government reserves the right to utilize other factors such as schedule and past performance on previous delivery orders, in the evaluation of delivery orders as determined necessary to meet requirements.

5. To remove the following duplicate clauses:

52.203-11 Certification and Disclosure Regarding Payments to Influence Certain Federal Transactions SEP 2007 52.204-3 Taxpayer Identification OCT 1998 52.209-5 Certification Regarding Responsibility Matters OCT 2015 52.215-6 Place of Performance OCT 1997 52.219-1 Small Business Program Representations OCT 2014

Q3. Is the citing the commercial packing clause for this solicitation the most cost effective method of packaging for this item?

The following have been modified:

EVALUATION FOR AWARD

Government reserves the right to utilize other factors such as schedule and past performance on previous delivery orders as determined necessary to meet requirements.
