About this file
This is a base access and personnel vetting notice issued by the 167th Air Wing (AW) detailing form submission requirements and gate access procedures for contractors and personnel visiting the facility.
The document specifies mandatory form completion requirements including: full birth name on line 1.3, all aliases and maiden names on line 1.4, complete Social Security number and date of birth on line 1.5, country of birth on line 1.6, and a wet signature (not digital) on line 1.10. All forms must be submitted no later than 10 business days prior to base arrival. The notice establishes that any individual arriving at the gate without pre-vetting clearance will be denied access, and this requirement applies to all vehicle passengers as well. Personnel from foreign countries must submit immigration paperwork and driver's licenses demonstrating legal work or residency authorization in the United States; failure to provide this documentation results in automatic denial of access. Naturalized U.S. citizens must provide copies of their naturalization certificates with background forms. The document emphasizes that individuals working in the USA on any visa type must carry their alien registration certificate or receipt card at all times in compliance with federal immigration law, with violations subject to fines up to $100 or imprisonment up to 30 days. Main company contractors are responsible for communicating these requirements to all personnel and are advised to distribute this information to prevent claims of non-notification. Inquiries regarding these procedures should be directed to MSgt Green or Natalie Clower at natalie.clower.1@us.af.mil.
