2. Description
This is a combination synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate, written solicitation will not be issued.
3. Synopsis
The 187th Fighter Wing (FW), Alabama Air National Guard, has a requirement for an Optimizing Human Weapon System Services (OHWS) program at the 187th Fighter Wing, Dannelly Field, Montgomery, AL, is to reduce the frequency and severity of neck and back pain injuries, reduce duty time lost, improve quality of life, enhance readiness, and support fighter aircrew retention. This program is described as pre-habilitation. Pre-habilitation involves prevention of disability and pain experienced as a result of fighter pilot operations through a program specifically designed to train and otherwise prepare aircrew for the rigorous demands of fighter aircraft. This is a non-personal healthcare services contract, as defined in
FAR 37.101.
Vendors must complete the attached SF1449 documentation for providing a proposal. Please review these documents in their entirety for a complete description of this requirement and required submissions for your proposal to be considered technically acceptable. All required attachments, documents, and information stated in the Instructions to Offerors must be included.
This requirement is a 100% Small Business set-aside requirement. The North American Industry Classification System (NAICS) Code is 621340 and the Size Standard for Small Business is $8.0M. The Service Contract Award Wage Determination 2015-4607 Revision 10, dated 18 June 2020, is applicable for this requirement.
The Government intends to issue a Firm Fixed-Price contract for these services, see Evaluation and Award Factors, Paragraph 4, for award basis. The Government reserves the right to cancel this solicitation, either before or after the response date. Proposals shall be valid until 30 October 2020.
PARTIAL OFFERS WILL NOT BE CONSIDERED. PARTIAL AWARDS WILL NOT BE MADE.
4. Evaluation and Award Factors The Contractor is required to be registered in the System of Awards Management (SAM) database, and maintain the registration throughout the life of the contract in order to receive payment for invoice(s).
Determination of this Firm Fixed Price award will be Best Value based on the following factors used to evaluate offers: A) Past Performance, B) Price and C) Technical.
A. Past performance shall be evaluated in two parts: 1. Relevancy and 2. Performance level based on responses received to the Past Performance Questionnaire. Past performance must reflect relevant, both in scope and capability, work for a period of up to five (5) years prior to this requirement’s solicitation date. Offerors must submit a minimum of one and maximum of two customers capable of providing a response to the 187th FW/ CONS office for the questionnaire. Respondents will be asked to rate the offeror’s past performance according to the following ratings:
1. Substantial Confidence: Met contractual requirements and exceeded requirements that benefit the end user.
2. Satisfactory Confidence: Met contractual requirements.
3. Limited Confidence: Did not meet some or all contractual requirements.
B. Price will contribute substantially to the selection decision. The 187th FW/CONS will evaluate all offers for full-time performance. However, based on available funding, responsive Contractors may be requested to provide a proposal for part-time performance. An award will be made in accordance with available funding.
C. The failure of an offeror to meet the requirements of a factor or sub-factor will result in an unacceptable rating. The Formal Care Plan, Sample Monthly Report and Staffing Plan shall be considered sub-factors of the Technical portion. Technical will receive one of the ratings listed below, in addition to the ratings of sub-factors:
1. Acceptable: Proposal clearly meets the requirements of the solicitation and specifications as outlined in the Performance Work Statement.
2. Unacceptable: The quote does not clearly meet the requirements of the solicitation or specifications as outlined in the Performance Work Statement.
The following ratings will be used for mandatory submittal items (Past Performance, Formal Care Plan, Sample Monthly Report and Staffing Plan) and contribute to the applicable rating (Past Performance or Technical):
1. Substantial Confidence: Meets contractual requirements and exceeds requirements that benefit the end user.
2. Satisfactory Confidence: Meets contractual requirements.
3. Limited Confidence: Does not meet some or all contractual requirements.
5. INSTRUCTIONS TO OFFERORS
Offerors must submit their proposals on the attached SF1449 documentation and include the following mandatory documents in support, not larger than the listed page limit:
