About this file
This is a Human Trafficking Attestation form required by Section 787.06 of the Florida Statutes for the County of Volusia in connection with Contract Number 26-B-73LS. The attestation serves as a compliance document that must be executed, renewed, or extended between nongovernmental entities and Volusia County as a governmental entity. The form requires a duly authorized officer or representative of the contracting entity to sign under penalty of perjury, attesting that the entity does not use coercion for labor or services. The attestation applies to the purchase of four (4) 2026 Kubota RTV-X900 diesel utility terrain vehicles for use by Volusia County Beach Safety on the beach throughout the year, equipped with specified features including 4x4 capability, oversize/offroad tires, and standard cargo beds.
The attestation defines coercion broadly to include physical force, unlawful restraint or confinement, debt bondage, document confiscation, financial harm, fraud, and the provision of controlled substances for exploitation purposes. By executing the form, the nongovernmental entity certifies compliance with statutory requirements and commits to immediately notifying Volusia County if it engages in coercive labor or services practices at any future time. The failure to maintain compliance would result in the inability to execute, renew, or extend contracts with the county. No specific pricing, funding sources, or contract term information is provided in this attestation document.

The Nongovernmental Entity acknowledges that Section 787.06(14), Florida Statutes, provides that when a contract is executed, renewed, or extended between a nongovernmental entity and a governmental entity, the nongovernmental entity must provide the governmental entity with an affidavit signed by an officer or a representative of the nongovernmental entity under penalty of perjury attesting that the nongovernmental entity does not use coercion for labor or services. For purposes of this requirement, “labor” means work of economic or financial value and “services” means any act committed at the behest of, under the supervision of, or for the benefit of another. The term, “services” includes, but is not limited to, forced marriage, servitude, or the removal of organs.

This signed attestation is provided to the Governmental Entity to comply with the statutory requirement.
If, at any time in the future, the Nongovernmental Entity does use coercion for labor or services, the Nongovernmental Entity will immediately notify the Governmental Entity and no contracts may be executed, renewed, or extended between the parties.
This attestation is made for the benefit of, and reliance by, the Governmental Entity.
Under penalties of perjury, I declare that I have read the foregoing statement and that the facts stated in it are true.
