About this file
This document contains specifications for State of Maine Vital Record Forms submitted by the Department of Health & Human Services, Office Services for Vital Records, with the specifications initially submitted in May 2006, revised in May 2010, and further revised in March 2012. The specifications detail requirements for blank safety paper forms used to produce vital records certificates. The finished documents are 8.5 by 11 inches and must be printed on 28-pound official vital record watermark paper containing multiple security features including a Fourdrinier true mill security style watermark, two-color invisible fluorescent security fibers, toner retention treatment, and full-family chemical sensitivity. The forms require three-color printing on the face: permanent black ink for text matter, PMS 287 blue for a high-resolution intaglio engraved border, and iridescent safety pantograph background with erasure-sensitive ink. The vendor must package 2,000 forms per carton with 500 forms per inner package, using 275-pound test RSC cartons standing 12 inches in height, with each carton clearly labeled with the document name, form number, and quantity. Delivery must occur under secure conditions with inside delivery to the specified Augusta, Maine address.
The document requires extensive security features and precautions including a high-resolution fine-line intaglio printed engraved border that vendors must match exactly, latent images of "V" and "R" embedded in the border design, a warning notice about alteration or erasure engraved into the border, engraved microprinting equivalent to LogoLine, and a permanent sequential letterpress printed control number in red ink on the certificate backside. All printing and storage must occur under secure conditions, all spoilage must be incinerated or security shredded, and vendors must use nickel plates for high-quality ink coverage with proof of such verification. No pricing, award dates, contract terms, set-asides, or incumbent information is specified in this document.
