The solicitation seeks vendors to supply vital records forms on specified paper stock with integrated security features. Bidders must submit samples of prior work produced on the requested paper stock that include all outlined security features and demonstrate the quality expected to be received. Award determination will be based on a weighted evaluation system: Quality accounts for 60 percent and is assessed through submitted samples of prior work and demonstrated ability to provide the product as specified; Delivery accounts for 5 percent and requires vendors to include their best delivery date for the first shipment of forms; and Cost accounts for 35 percent and must include all expenses for printing, shipping, and handling. Bidders are responsible for providing all supporting information necessary to respond to each evaluation criterion.
