Attachment 8, Non-VISN 15 Beneficiaries

This agreement provides for placement of patients by VISN 15 VA Medical Centers. VISN 15 VA Medical Centers include:

1. Station 657, St. Louis, Missouri and its catchment areas.

2. Station 589, Kansas City, Missouri and its catchment areas.

3. Station 657A4, Poplar Bluff, Missouri and its catchment areas.

4. Station 589A4, Columbia, Missouri and its catchment areas.

5. Station 657A5, Marion, Illinois and its catchment areas.

6. Station 589A5, Topeka, Kansas and its catchment areas.

7. Station 589A6, Leavenworth, Kansas and its catchment areas.

8. Station 589A7, Wichita, Kansas and its catchment areas.

Non-VISN 15 located outside of the catchment areas above may occasionally need to place beneficiaries at the contracted CNH facility, and are responsible for all oversight of the beneficiary, to include care, invoices, payments and discharges. The placing VA may coordinate with the VISN 15 VAMC CNH Team for assistance.

Any such placement is not under the terms of this agreement.

The placement may only become a placement under the terms of this agreement upon transfer of oversight of the patient care from the Non-VISN 15 VAMC to the applicable VISN 15 VAMC. The respective CNH Team coordinators will consult with the CNH facility regarding any transfer of oversight, which may include adjusting rates, care coordination, contact information and invoicing procedures.

It is the contractor’s responsibility to verify placements with the CNH Team Social Workers, Nursing or COR staff. See Sections B4, 4.2 and 4.3.
