About this file

This is a Vendor Questions Worksheet for RFP 26*2295 issued by the State of Michigan for the BSBP-BEP Refrigeration Replacements project involving 43 units. The document serves as a standardized template requiring bidders to provide detailed responses across thirteen categories of information and certifications. Bidders must submit comprehensive contact information, company background details including legal business structure and formation state, and documentation of relevant experience from the past five years demonstrating capability to manage similar projects. The worksheet requires submission of at least three detailed project references with specific information regarding project scope, company role, budget, timeline, and results achieved. Additionally, bidders must disclose any history of debarment, suspension, litigation against the State of Michigan, contract defaults or terminations for cause within the past five years, and provide gross annual sales figures for the preceding five years.

The worksheet mandates that bidders affirm compliance with all attached contract terms and insurance requirements, with any exceptions requiring formal documentation according to the RFP's evaluation process guidelines. Bidders must certify compliance with Michigan labor, antidiscrimination, environmental, and prevailing wage laws; disclose any violations received within the past five years; and provide certifications regarding employee classification, abusive labor practices, Michigan business tax filing, Iran-linked business status, clean corporate citizenship, and state tax payment obligations. The document also requires bidders to indicate Michigan Voluntary Protection Program recognition status, report Michigan-based employment figures and wages, confirm authorization for state verification of all submitted information, and commit to responding by established deadlines to all state requests including clarification requests and notices of deficiency.
