About this file
The document is a Vendor Questionnaire related to a pesticide disposal event, seeking detailed information from potential vendors about their capabilities for managing agricultural pesticide collection and disposal. The questionnaire covers multiple aspects of event planning and execution, including vendor experience, venue requirements, event setup, equipment provisions, disposal methods, reporting capabilities, logistical considerations, and potential pricing structures. The document appears to be seeking comprehensive information to evaluate vendor qualifications for a large-scale pesticide disposal project involving multiple counties.
The questionnaire requests specific details about vendor pricing, including whether rates vary by quantity collected, additional fees, and the ability to conduct mobile collection at farms without dedicated event spaces. Key areas of interest include the vendor's capacity to provide complete event infrastructure (forklifts, scales, signage, personal protective equipment), time required for event setup, advanced planning needs, legal reporting capabilities, and potential permitting requirements for mobile collection services. The document suggests the project involves collecting agricultural pesticides from multiple farms, with previous events serving as a reference point for anticipated quantities and logistical complexity.

Attachment III Vendor Questionnaire
| Question |
| Vendor Response |
1. Have you been the vendor for this type of pesticide disposal events before?
2. Is your company able to provide or obtain venues to hold these events? If vendor is not able to provide a venue, what are the requirements needed in a venue for these events?
3. How much time does it take to set up a disposal event?
4. Can your company provide all equipment and manpower required for the event if all the grower does is pull their vehicle up with their pesticide containers in it? (i.e. Forklifts, scale, signage, road cones, tarps, PPE, pallets, trash cans, boxes, etc.) Are there other items that are either beneficial or needed you will not provide such as port-a-potties, pop-up tents, etc?
5. Do any of these requirements from RFI Section E. Concept of Project create a hardship or roadblock to implement this event as a vendor?
6. What method of disposal does your company use?
7. What requirements may prevent you from bidding on a resulting solicitation? Please provide suggestions about what additional project details will help in creating a bid.
8. How much advanced notice does the vendor need to plan for and implement a disposal event?
9. Could your company offer as a service the drafting and filing of all the legally required reports, forms, and other paperwork?
10. Please provide a pricing structure that would be a reasonable example for a large-scale event. See attachment II as an example of quantities collected during previous events. Events could be one day or multiple days. Does the rate change depending on the quantity collected? Are there any other fees you require to contract for this event?
11. Is the vendor capable of traveling to pre-identified farms in counties where there isn’t space to host a full event? What are the permitting requirements for this service?
