About this file

This document is a Vendor Questions Worksheet for Request for Proposal No. 180000000088 issued by the State of Michigan Department of Transportation for the Official Michigan Transportation Map project. The worksheet serves as a standardized questionnaire that bidders must complete as part of their proposal submission. The document does not specify project quantities, response dates, due dates, site visits, bidders' conferences, award dates, or contract terms, as it functions primarily as an information-gathering instrument rather than a project specification document.

The worksheet requires bidders to provide detailed responses across six major categories: company contact information and authorized contract signatories; comprehensive company background information including legal business name, incorporation state, years in business, organizational structure changes, debarment history, litigation against Michigan, contract defaults or terminations within the past five years, and gross annual sales for the preceding five years; qualified disabled veteran business status with required documentation; confirmation of non-participation in RFP development or evaluation; relevant experience with the State of Michigan including specific contract numbers from the past ten years; and at least three relevant experiences from the past five years demonstrating capability to manage similar contracts, including project descriptions, company roles, relevance to the RFP, dollar values, timelines, and results. The worksheet does not address pricing, set-asides, incumbent information, or funding sources, as these elements would be contained in accompanying RFP documents.
