About this file

This is a Prime Contractor Response Qualification Form for the Bayport North Wildlife Management Area Woody Biomass Piles Removal project in Washington County/Bayport (Solicitation #2025-443). The form outlines qualification requirements for prime contractors bidding on large-scale woody biomass removal work. Contractors must demonstrate completion of at least one similar project valued at $50,000 or greater since August 1, 2019, as a prime contractor rather than a subcontractor.

The qualification form requires contractors to verify they have not been found in default or had contracts terminated for cause in the last 36 months, nor received any willful or repeated OSHA safety citations with final orders issued in the same timeframe. Contractors must provide detailed information about previous qualifying projects, including property owner contacts, project size in acres, contract amounts, and completion dates. The State reserves the right to contact references and request additional project information to evaluate contractor responsibility based on factors like project milestone achievement, communication, supervision, and compliance with prevailing wage laws.
