About this file

This document is a Vendor Authorization Request form for domestic vendors seeking to do business with ASRC Federal and its subsidiaries through the Costpoint system. The form serves as a vendor registration and authorization tool that collects taxpayer identification information, business classification data, and contact details required for payment processing and federal contracting compliance.

The form requires vendors to provide a nine-digit Taxpayer Identification Number (EIN, SSN, or ITIN), legal business name, business address, remittance address, and contact information. Vendors must identify their entity type (sole proprietor, LLC, corporation, partnership, trust, or other), entity classification (C-corporation, S-corporation, partnership, etc.), and business size (small business, large business, foreign-owned, non-profit, or other). The form includes sections for vendors to report optional identifiers such as UEI number, DUNS number, and CAGE code. Vendors are required to disclose any applicable business classifications, including SBA certifications (8(a), HUBZone, WOSB, Service-Disabled Veteran-Owned, etc.). ASRC Federal defaults to Net 45 payment terms for large businesses and Net 30 for small businesses unless otherwise specified in the sales contract or purchase order. Electronic payments are facilitated through Corpay Payment Automation, though vendors must receive one paper check prior to enrollment. The form includes a certification section where an authorized representative must sign using a certified digital signature or wet ink signature, certifying the accuracy of the taxpayer identification number and confirming U.S. person status. A separate "Modification to Existing Vendor Records" form is provided for vendors to update address, contact information, legal name, or tax identification number changes.

Vendor Authorization Request - Domestic Instructions PAGE 1

For ASRC Federal and Subsidiary Vendors ONLY System: Costpoint

Instructions For General Instructions refer to the IRS Form W-9 located on the IRS website (www.irs.gov).

ASRC Federal defaults to Net 45 payment terms for large business and Net 30 for small business, unless stated otherwise in the sales contract/purchase order.

ASRC Federal and its Subsidiaries (ASRC) may award procurement to the seller where the costs will be charged to a U.S. government prime or subcontract.

If so, the seller is advised that the U.S. government may impose a penalty against a firm misrepresenting its business size and/or disadvantaged status for the purposes of obtaining procurement that is to be included as part, or all of a goal contained in ASRC’s Subcontracting plan. Eligibility as a small business is based on the regulations issued by the Small Business Administration in CFR 13, Part 121 of the SBA Rules and Regulations and FAR 52.219-1.

Electronic Payments ASRC Federal has partnered with Corpay Payment Automation to facilitate electronic payments to our Vendors. To receive electronic payments, you must enroll with Corpay Payment Automation.

Note: You are required to receive one paper check prior to enrolling in Corpay Payment Automation
