About this file

This is a Vendor Affidavit regarding the Use of Coercion for Labor and Services required by the School Board of Hillsborough County Public Schools (HCPS) in Florida for Emage Medical, a vendor seeking to execute, renew, or extend a contract with the school district. The affidavit is mandated under Section 787.06(13) of the Florida Statutes and applies to all nongovernmental entities contracting with governmental agencies. The vendor must certify under penalty of perjury that the company does not engage in human trafficking or labor coercion practices as defined by state law. The affidavit requires an authorized representative or officer of the contractor to sign the document, providing their name, title, and date of execution.

The affidavit specifically requires the contractor to affirm that it does not use or threaten physical force against persons; restrain, isolate, or confine individuals without lawful authority; use fraudulent lending or credit methods to create debt through pledged labor; destroy or confiscate government identification or immigration documents; cause financial harm; entice individuals through fraud or deceit; or provide controlled substances for exploitation. This compliance document serves as a prerequisite to contract award and must accompany the bid submission or contract renewal documentation for HCPS's Emage Medical equipment and services procurement.
