About this file

This is a vendor affidavit document required by Hillsborough County Public Schools (HCPS) in Florida for contractors executing, renewing, or extending contracts with the school district. The affidavit mandates that authorized representatives of contracting entities certify under penalty of perjury that their organizations do not engage in coercion for labor or services as defined under Florida Statutes Section 787.06(13). The certification specifically addresses prohibited practices including the use of physical force or threats, unlawful restraint or confinement, exploitative debt practices, possession or destruction of identification documents, threats of financial harm, fraudulent enticement, and provision of controlled substances for exploitation purposes.

This document is a compliance requirement tied to the District's Invitation to Quote for Biliteracy Engagement Materials, which seeks vendors to provide services enhancing English language development, supporting access to grade-level content, and strengthening family engagement. All nongovernmental entities bidding on or contracted with HCPS must complete and submit this affidavit as part of their proposal or contract documentation. The affidavit must be signed by an authorized officer or representative of the contracting company, with spaces provided for signature, printed name, title, and date. No pricing, set-asides, renewal terms, or other contract-specific details are contained within this affidavit form, as it serves solely as a human trafficking and labor coercion compliance certification required by Florida law for all government contracts.
