About this file
This is FEMA Form 089-4B, Appendix E, a required template for submitting a Five-Year Security Capital and Operational Sustainment Plan as part of the Transit Security Grant Program (TSGP). The form is used by applicants requesting Operational Packages (OPacks) to demonstrate how they will implement capital projects and sustain operational investments and capabilities after grant funding has been expended. Requests for OPacks will not be funded if the applicant does not include a Security Capital and Operational Sustainment Plan. The form is authorized under Section 1406, Title XIV of the Implementing Recommendations of the 9/11 Commission Act of 2007 (6 U.S.C. § 1135) and is controlled as Sensitive Security Information under 49 CFR parts 15 and 1520, restricting disclosure to authorized personnel with a documented "need to know." The OMB control number is 1660-0112, with an expiration date of April 30, 2023, and the estimated public reporting burden is 5 hours per response.
The form requires applicants to provide narrative responses organized into six sections: an abstract narrative of the overall five-year plan, followed by detailed year-by-year outlines for years one through five. All submissions must be submitted electronically through ND Grants as file attachments using the naming convention "UASI Area_Applicant_IJ Number" (e.g., Chicago_XYZTA_Plan_1). If submitting multiple encrypted documents, applicants must use the same password for all files. The form includes header fields for investment details such as date, applicant name, investment name, region and urban areas impacted, investment amount, and scalability designation. Additional space is provided at the end of the form for extended responses to each section.
