About this file

This is a Data Item Description (DID) template for test/inspection reports used in federal contracting to document test and inspection results, findings, and analyses for system compliance evaluation.

The DID establishes mandatory format and content requirements for test/inspection reports applicable to engineering (developmental), preliminary qualification, qualification, and acceptance testing activities. Reports must include a cover page with report date, number, contractor information, contract details, test type, item identification, test period, requiring government activity, and security classification; a table of contents identifying all major sections and illustrations; an introduction specifying test/inspection objectives, complete item identification (nomenclature, stock number, model/part/serial numbers, item type, manufacturing date, and applicable engineering changes), and test/inspection requirements correlated to contractual performance criteria; a summary discussing significant results, observations, conclusions, recommendations, proposed corrective actions, and identified deviations; and reference documents including prior test reports, plans, procedures, and certifications. The body of the report must contain complete identification of test equipment with nomenclature, model, serial number, manufacturer, calibration status, and accuracy data; detailed description of facility installation and physical setup including item location, equipment positioning, sensor placement, interconnections, and power requirements; complete procedures describing item selection, testing sequence, and control conditions; actual recorded data and test/inspection results with matrices comparing achievements against objectives; and conclusions distinguishing between objective and subjective findings. The report must include three authenticated certifications: that testing was performed in accordance with applicable plans and procedures with true results, that previously validated requirements not tested are identified with validation methodology, and that the item passed or failed acceptability requirements. Appendices may include detailed data, drawings, photographs, and referenced documentation too voluminous for the main report. Contractor format is acceptable provided all required information is organized logically and comprehensively.
