SCOPE OF WORK
1. GENERAL REQUIREMENTS
General
The work is located at 100 Dashields Lock Road, Coraopolis, PA 15108-5417. The site of the work may be reached from state and local roads and streets. The purpose of this contract is to replace the EPDM sheet roof systems of two Lock buildings. One building is located on the Middle Wall and one building is located on the River Wall of the Lock. All work shall be done in a professional manner and in accordance with industry standards. The roof systems shall be replaced as specified herein and provide a water tight seal upon completion of the work.
The contractor shall install anchor points for use in a personal fall arrest system to permit safe personnel access to the entire roof surfaces. The contractor shall install two roof-mounted, fall protection rooftop anchorages, tie-off points on the main Middle Wall roof and two more on the River Wall roof. Each anchorage shall be rated for one person. These tie-off points shall be mounted on top of the membrane for ease of inspection. The four locations are shown on SK-1.1 and SK-1.2 within Attachment 5. These anchorage points can be utilized during performance of the work and must be left permanently installed upon completion of the roof replacement.
The only direct access from the Land Wall to the Middle and River Walls are the walkways over the lock gates, which are Aluminum grating. The Contractor is responsible for getting materials and equipment over to the buildings. All instances where Government materials, supplies, property, and/or equipment are damaged, shall be reported immediately to the Contracting Officer (KO) and Lockmaster. All damage to existing government property as a result of the Contractor’s work shall be repaired or replaced to the satisfaction of the Contracting Officer.
1.1.1. Middle Wall Building Roof
The main roof is approximately 30 feet by 18 feet, or 540 square feet. There is also a small roof, above the 1st floor entrance door, with approximate dimensions of 4 feet by 5.5 feet, which shall also be replaced. New flashing, vent covers, and caps shall be installed on pipe vents protruding from roof surface. The existing roof ventilator shall be removed and reinstalled. The antennas shall be supported in place and remain functional during the roof replacement. Project personnel will designate a laydown area next to the worksite which can be used to store materials. All debris to include existing roofing materials must be removed from the lock walls daily.
Removal and or abatement of lead based paint (LBP)/lead dust is anticipated. The building shall be tested for lead (LBP/lead dust) prior to the commencement of demolition activities. If sampling confirms the presence of LBP/lead dust, a written lead compliance plan shall be required.
1.1.2. River Wall Building Roof
This roof is approximately 61 feet by 18 feet, or 1100 square feet. New flashing, vent covers, and caps shall be installed on pipe vents protruding from the roof surfaces. The wooden elevated cover with approximate dimensions of 9 feet by 16 feet shall be removed and replaced with wooden joists and sheeting. The two roof ventilators shall be removed and disposed of by the contractor. The antennas shall be supported in place and remain functional during the roof replacement. Project personnel will designate a laydown area next to the worksite which can be used to store materials. All debris to include existing roofing materials must be removed from the lock walls daily.
1.1.3. Labor and materials
The contractor shall provide all labor and materials necessary to replace the existing built up roof, roof drains, and associated items. Details of the building construction are shown on the attached reference drawings. The work shall include removal and disposal of all roofing materials over the entire roof down to its substrates of existing concrete, steel decking, or wood deck, including all flashing, roof drains, flashing around base of the stack, and installation of new tapered insulation, flashing, roof drains, and all other work to furnish all equipment, labor, layout of work, supervision, and materials necessary to complete the work as shown and as specified. The Contractor shall be responsible for transport of materials to the site and to prevent demolished materials from entering the river. All material provided in the performance of this contract shall be unused and meet the requirements of the specifications. The antennas on the existing roofs shall be temporarily supported during replacement of the roofs. They shall remain connected and functional during the performance of the work.
1.1.4. Existing items
The EPDM shall have a proper waterproof seal around all existing items that protrude thru or from the roof.
1.1.5. Protection
The antennas shall be cribbed so the contractor can work underneath, removing roofing material and installing the new roofing. The antennas, and any other surface bearing items, shall have paver blocks on a layer of reinforced membrane or walkpad applied over the completed roof membrane to protect the new roof, as discussed in Isolated Paver Blocks.
Electric or communication lines will stay in place and should not be disturbed.
1.1.6. Existing roofs
Refer to photos within Attachment 2 for existing roof conditions. The Middle Wall roof is 540 square feet and its substrate consists of steel decking on steel C joints. The River Wall roof is 1100 square feet and its substrate consists of concrete, except an area, consisting of wood, of approximately 144 square feet. The Contractor shall verify dimensions and clearances in the field before beginning work. This includes a lip of approximately 18 inches around the perimeter of each roof. Roof elevation is approximately 20 feet above each lock wall. The existing roof system is about 4 inches thick and consists of the following layers over the concrete substrate:
o Mopped asphalt o 2 ply built up roof o 0.5-inch wood fiber board o 3.5 to 1.5-inches of tapered insulation

In addition to the detailed requirements included in the provisions of this contract, all work shall comply with the most recent edition of the USACE Engineering Manual (EM) 385-1-1, “Safety and Health Requirements Manual”, and all applicable federal, state, and local laws, ordinances, criteria, rules, and regulations. If there are any standards that require interpretation, submit matters to the appropriate administrative agency for resolution before the commencement of work. Where the requirements herein, applicable laws, criteria, ordinances, regulations, and referenced documents vary, the most stringent requirements govern.
The Superintendent, QC Manager, and SSHO are subject to dismissal if any of the required regulatory requirements are not being effectively carried out. If dismissal occurs, project work will be stopped and no work shall proceed until a suitable replacement is approved and the appropriate requirements are assured to be effectively carried out.
The latest version of the EM 385-1-1 is available online at:
https://www.publications.usace.army.mil/Portals/76/Publications/EngineerManuals/EM_385- 1-1.pdf?ver=2015-04-09-142531-467.
1.2.1. Accident Prevention Plan (APP)
An Accident Prevention Plan (APP) is a safety and health policy and program document that shall be contract- and/or job-specific. The APP shall be developed by the Contractor and shall interface with the Contractor's overall Safety and Health Program, such that any portions of the Contractor's overall program referenced in the APP shall be included as appropriate.
The Contractor shall develop the APP in accordance with EM 385-1-1, Appendix A, and shall address each of the elements/sub-elements in the outline contained in the appendix in the order that they are provided in the manual, and as supplemented herein. If an item is not applicable because of the nature of the work to be performed, the Contractor shall state this exception and provide a justification. The APP shall be written in English by the Prime Contractor and shall articulate the specific work, work processes, equipment to be used, and hazards pertaining to the contract. The non-mandatory APP Checklist, Form A-2 of the EM 385-1-1, may be used to ensure each area of required information has been provided as part of the APP to the Contracting Officer.
The Contractor's APP shall be job-specific and must include work to be performed by subcontractors. It is understood that the APP is submitted prior to work starting and some of the project-specific information is not yet known. If sub-contractors and/or suppliers are not known at the time of initial APP submission, the Contractor shall include the following statement in their APP: "The subcontractors for the following defined features of work (DFOW)/activities are not known at this time, but additional information will be submitted to the APP for acceptance prior to the start of any activities listed." All known information shall be provided and additional information submitted to the Contracting Officer for acceptance as it becomes known.
Once accepted by the Contracting Officer, the APP and all attachments will be enforced as part of the contract. Disregarding provisions of this contract or the accepted APP is a cause for stopping of that portion of the work at the direction of the Contracting Officer until the matter has been rectified.
A copy of the accepted APP shall be available on the worksite at all times, and each sub-contractor shall be provided a copy to comply with. A signature log shall be completed by each sub-contractor foreman attesting that they have read and understand the APP.
The Contractor shall provide on-going evaluations of the APP throughout the life of the project. Changes, revisions, and updates to the APP shall be reviewed and accepted by the Contracting Officer, project superintendent, SSHO, and Quality Control Manger. An APP template is provided in Attachment 3.
1.2.2. Risk Management
Risk management is a business process that includes the identification, assessment, and prioritization of risks, followed by coordinated and economical application of resources to minimize, monitor, and control the probability and/or impact of unfortunate events to an acceptable level. A risk management assessment tool, such as activity hazard analysis (AHA), job safety analysis (JSA), or job hazard analysis (JHA) shall be developed by the contractor performing the work for each activity, task, or defined feature of work (DFOW).
The assessment shall identify the work sequence, specific anticipated hazards, site conditions, equipment, materials, personnel, and the control measures to be implemented to control the hazards. A Risk Assessment Code (RAC) shall be assigned to each job step based on the residual risk remaining after controls are implemented. A RAC shall then be assigned to the overall activity consistent with the highest RAC identified for the job steps. If developed by a subcontractor, the Prime Contractor shall review and approve the assessment before submission to the Contracting Officer. The Risk Assessment shall be signed by the SSHO, Superintendent, QC Manager, and any subcontracting Foreman's performing the work. If Qualified or Competent Persons are required for a particular activity, they shall be identified and included in the assessment, and their competency/qualification identified. Work shall not begin until the assessment has been accepted by the Contracting Officer and discussed with all engaged in the activity.
These risk assessment tools are considered living documents and shall be updated as needed. If the RAC increases from what was originally accepted by the Contracting Officer, the assessment shall be re-submitted for acceptance prior to work proceeding. Changes that do not increase the RAC are not required to be resubmitted for acceptance.
The risk assessment shall be developed in accordance with EM 385-1-1 Section 1, or as specified by the Contracting Officer. Risk assessments shall be submitted for review at least 15 working days prior to the start of the activity, task, or defined feature of work is set to begin. The Government reserves the right to require the Contractor to revise and resubmit any documents if they fail to effectively identify the work sequence, specific anticipated hazards, site conditions, equipment, materials, personnel, and the control measures to be implemented.
Each employee performing work as part of an activity, task, or defined feature of work shall review the risk assessment for the activity and sign the signature log designated for the assessment prior to beginning work. A signature log shall be maintained for each risk assessment.
Completed risk assessments need not be submitted at the time of the APP submission, but the Contractor shall identify each activity, task, or DFOW that will require a risk assessment to be completed at the time of submission.
1.2.3. Implementation of Safety and Occupational Health (SOH) Program The Contractor shall employ a minimum of one Competent Person at each project site to function as the primary Site Safety and Health Officer (SSHO) to implement and administer the Contractor’s safety program and the Government-accepted Accident Prevention Plan
(APP). The SSHO is a full-time responsibility and shall be an employee other than the supervisor [unless specified differently in the contract and coordinated with the local SOH Office], that reports directly to a senior project, or corporate, official. The SSHO shall be present at the project site and be located so they have full mobility and reasonable access to all major work operations during the shift. The SSHO shall meet all training and experience qualifications as outlined in EM 385-1-1 Section 01, Paragraph A.17.
For projects with multiple shifts, or in instances where the SSHO is off-site for a period longer than 24 hours, an Alternate SSHO shall be assigned to ensure coverage for the project at all times work activities are conducted. The alternate SSHO shall meet the same requirements and assume the responsibilities of the primary SSHO. The alternate SSHO shall be identified in the applicable AHAs for the activities they are responsible for overseeing.
When the primary SSHO is temporarily, up to 24 hours, off-site, a Designated Representative (DR) may be used in lieu of an Alternate SSHO. The DR shall be on the project site at all times when work is being performed in the absence of the primary SSHO.
DRs can be collateral duty safety personnel, with safety duties assigned in addition to their full-time occupation. DRs shall perform safety program tasks as designated by the SSHO, and report all findings to the SSHO. DRs shall also be appointed when activities, tasks, or DFOWs contain multiple sites and are more than 45 minutes travel time from the primary SSHO's duty location. The DR shall be identified in the applicable AHAs for the activities they are responsible for overseeing.
Note: DRs may not be assigned to projects or activities that have a risk assessment code (RAC), as identified on the AHA for the activity, level of high or extremely high.
