PUBLIC WORKS DUMPSTER REQUEST FORM

AND INSTRUCTIONS

INTRODUCTION: This form is to be used by DPW customers to request refuse dumpsters and roll-offs. It includes specific instructions and caveats to assure that the service requested is provided on a timely basis. The customer assumes various responsibilities as noted herein.

Read ALL instructions carefully, incorrectly filled out forms will cause delays.

Fill out request form and worksheet.

Email complete forms to COR no later than 15-working days prior to requireddate.

Changes to requests will also require 5 days to process.

COR will confirm request once funding, locations, schedules, and quantities have been verified.

DO NOT contact the Contractor or their employees directly; you could commit an unauthorized purchase or commitment. Contractor will not honor on-site requests.

Units/Customer funding will be coordinated through your supporting Budget Officeat the Resource Management Office. No request will be acted on until funding is certified.

Dumpsters will be placed to serve the maximum number of customers possible. One dumpster pick up for every thirty five soldiers/civilians. All temporary dumpsters will be serviced a minimum of 3 pickups per dumpster or more if requested by the customer. No services will be provided on Sundays or Holidays. Saturdays are by exception only as determined by the COR.

Refuse dumpster locations at Dona Ana, Oro Grande, and McGregor are subject to the direction for placement of the camp facility managers, DPTMS and/or RangeControl.

Please provide military coordinates or a map for proper location of refuse dumpsters TRAINING AREAS REQUESTS: Range Division Logistics does not request dumpsters to service Units. Units are required to use this form to request specific services through their Commander, Executive Officer or S-4.

Units must request refuse dumpsters at their respective training area locations. Use of other dumpsters in lieu of Unit requested dumpsters is not allowed.

b. The bin coordination will be reviewed for approval. If the dumpstercannot be staged at the coordinate given, then an alternate location will beused.

c. Units will not dispose of any unused flameless rations heaters (FRH) for

DPW-E. No pallets or large wood items, such as RR-ties, doors, plywood, 2X4, etc. Contractor will not pick up such items. Place wood pallets on one side of the dumpster for separate pickup. Allow room for service truck to reach containers.

d. Any questions pertaining to what can be allowed to be placed into the brown dumpsters, please contact the COR or ACOR.

e. Units will not combine ordnance with solid waste refuse. All ordnance material will be turned in accordance with the ammunition requirements in Chapter 3. Page 8, Explosive Ordnance Disposal Operations.
