About this file

This document is a Taxpayer Identification Number (TIN) form issued by the State of Illinois Chief Procurement Office under the General Services division. It is a standard form (Form A, Version 15.2a) used in conjunction with Invitation for Bid (IFB) or Request for Proposal (RFP) solicitations to collect taxpayer identification information from vendors and contractors doing business with the state.

The form requires vendors to provide either a Social Security Number (for individuals and sole proprietors) or an Employer Identification Number (for businesses, partnerships, corporations, and other entities). Vendors must certify their correct taxpayer identification status and confirm they are not subject to backup withholding by the Internal Revenue Service, or provide documentation of their exemption or release from backup withholding requirements. The form includes checkboxes for vendors to identify their legal business structure, including individual, sole proprietor, partnership, corporation, limited liability company, tax-exempt organization, governmental entity, and specialized categories such as pharmacies and legal services corporations. This form serves as a prerequisite documentation requirement for state contract awards and must be completed and signed by an authorized representative of the bidding or proposing entity.
