This is a Performance Work Statement (PWS) for the delivery, installation, functional testing, removal, and disposal of medical exam/procedure tables for the Crownpoint Service Unit (IHS). The contractor must deliver, install, and place twenty-two (22) new medical exam/procedure tables—eighteen (18) at Crownpoint Healthcare Facility and four (4) at Thoreau Health Clinic. Simultaneously, the contractor must remove and properly dispose of twenty-two (22) existing medical exam/procedure tables in accordance with applicable federal, state, and local regulations. All work must be performed in compliance with manufacturer installation requirements, OSHA standards, federal and state environmental regulations, facility infection prevention requirements, and Joint Commission healthcare facility standards.

The statement of work comprises seven specific tasks: (1) Project Coordination, including designation of a single point of contact and provision of a detailed installation schedule at least ten business days before delivery; (2) Delivery Services requiring on-time delivery to designated rooms with protection of facility infrastructure and maintenance of insurance coverage; (3) Installation and Setup involving unpacking, assembly, positioning, leveling, cleaning, and verification of accessories; (4) Functional Testing and Verification including manufacturer-recommended operational checks and verification of all functions and accessories; (5) Removal of Existing Equipment requiring disconnection and transport of existing tables while minimizing facility disruption; (6) Disposal of Existing Equipment in compliance with all applicable regulations with documentation; and (7) Site Cleanup requiring removal of all packaging materials and restoration of areas to pre-installation conditions. The contractor must complete all work within a specified number of calendar days (to be filled in) after receipt of award or notice to proceed. Government acceptance requires completion of delivery, installation of all tables, successful operational testing, removal and disposal of existing equipment, site cleanup, and submission of all required documentation. Required deliverables include Project Schedule, Delivery Receipt, Installation Completion Report, Functional Testing Checklist, Equipment Removal Log, Certificate of Disposal/Recycling, and Final Project Completion Report. The contractor must provide all labor, supervision, equipment, transportation, materials, tools, and incidentals necessary to complete the work. Installation and new parts warranty shall remain in effect for one year from final acceptance, with the contractor responsible for providing all manufacturer warranty information and operation/maintenance manuals.
