About this file
This document is a Pre-Quote Conference Presentation for a state and local contract opportunity titled "T1057- WIC Infant Formula Rebate: DOH" issued by the State of New Jersey. The purpose of this conference is to address procedural questions regarding the bid solicitation and quote submission requirements. The contract is for the provision of infant formula rebate services for the WIC program, and vendors must submit quotes by 2pm on January 25, 2023. The contract includes a scope of work, general contract terms, data security requirements, and evaluation criteria. Vendors must register in the NJSTART system and submit quotes addressing all mandatory requirements, including completing the provided price sheet without any changes or additions.
The price sheet requires vendors to specify a rebate and brand name for a single milk-based infant formula product in concentrated liquid, powdered, and ready-to-feed forms. The rebate will be calculated as a percentage of the manufacturer's lowest national wholesale price per unit. The contract is open to all qualified vendors, and there are no set-aside requirements mentioned. The presentation also cautions vendors against including any additional terms or conditions that conflict with the state's standard contract terms.
