About this file
This is an Application Response Form issued by the State of Maine, Department of Health and Human Services, for RFA #202608134 to solicit providers of Supervised Visitation Services for children in foster care or involved in child protection. Applicants may propose services in one or more of eight districts spanning Maine's counties, with reimbursement rates varying by district ($80 per unit in Districts 1 and 2; $70 per unit in Districts 3 through 8). The application requests detailed information on organizational qualifications, experience providing supervised visitation services, trauma-informed and culturally responsive approaches, capacity to serve specialized populations including infants and children with disabilities, and ability to support healthy parent-child interactions while ensuring child safety. Applicants must submit organizational charts, job descriptions, staffing plans, evidence-informed parenting models, training plans, implementation timelines, and three project references demonstrating relevant experience. The resulting contract will be a Standard Service Contract, and applicants' pricing structures remain firm for 180 days from bid opening.
The Department establishes all-inclusive reimbursement rates per unit (one hour equals one unit) and will not reimburse start-up costs. Applicants must designate whether they are Small Agencies (five or fewer staff conducting supervised visits) or Large Agencies (more than five staff), with small agencies exempt from facility and transportation requirements if not feasible. Small agencies must provide three years of balance sheets and income statements; large agencies must submit three years of audited or reviewed financial statements from a certified public accountant. All awarded providers must obtain general liability, professional liability, and other relevant liability insurance evidenced by an ACORD certificate. The application requires certification that the applicant and any subcontractors are not debarred, suspended, or otherwise ineligible; have not been convicted of fraud or related offenses within three years; and are not foreign adversary entities or on prohibited company lists.
