About this file

This is a Surplus Material Evaluation Form (v1.2) used to document and evaluate surplus items being offered in federal contract proposals in compliance with FAR 11.302(b).

The form requires offerors to provide comprehensive information across four main sections. Section A requests item identification details including National Stock Number (NSN), part number, drawing revision, and manufacturer. Section B establishes the surplus provenance, requiring the Federal Supply Code for Manufacturer (FSCM), original manufacturer contract number, source of subsequent purchase, DLA sale information, drawing availability with revision dates, and complete history of storage, packaging, rework, reconditioning, repair, or other modifications since initial government or manufacturer sale. Section C specifies warranty terms, with a presumed 12-month warranty period per FAR 52.246-17 unless otherwise stated. Section D classifies the surplus condition into four categories: (1) new and unused with no signs of deterioration; (2) new and reconditioned with replaced gaskets, seals, or O-rings per technical orders; (3) new and modified with component replacements for configuration updates; or (4) used and overhauled to serviceable status per applicable technical specifications. For categories (2), (3), and (4), offerors must identify the source and provide detailed summaries of reconditioning, modification, or overhaul work performed, along with relevant technical order compliance documentation. The form requires completion by the Source of Surplus (identified by company name and CAGE code) with signature and date from the SOS Releasing Official, and mandates two attachments: a copy of the original packing label and a Request for Quote Form.
