About this file

This is a Supplemental Bidder Attestation form issued by the State of Rhode Island Division of Purchases in accordance with R.I. Gen. Laws 37-2-13.1(b). The form requires bidders and their parent corporations, subsidiaries, affiliates, and subcontractors to certify that they do not have conflicts of interest with any official, officer, or agency overseeing the Request for Proposal and that they have not materially participated in or been consulted regarding the requirements, technical aspects, or formation of the RFP. The attestation also requires certification that the RFP does not relate to any audit, examination, independent verification, review, or evaluation of the bidder's work, financials, or operations performed on behalf of the State of Rhode Island or its officials or agencies. The form was revised on November 29, 2024.

The authorized signatory must complete all required fields including the vendor or entity name, date, authorized signature, solicitation number and title, and printed name of the authorized representative. The form clarifies that being a current or past vendor to the State does not automatically create a conflict of interest and that formal responses to a prior Request for Information do not constitute participation or consultation that would disqualify a vendor from bidding on a subsequent RFP.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the

State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
