About this file

This is a Subcontractor Summary worksheet instruction document issued by the City of St. Petersburg, Florida, for use in response to a solicitation for Police Department Ballistic Door Installation. The worksheet requires prime contractors to disclose all intended subcontractors and certify whether subcontractors will be used on the project. For this shelter-related project, up to 80 percent of the contract work may be subcontracted. The document requires bidders to provide complete legal names, primary business addresses, contact information for project supervisors, trade classifications, and subcontract amounts for each intended subcontractor. Subcontractors are classified as either first or second tier, with restrictions limiting multiple-tier subcontracting unless otherwise specified in contract documents.

Once a prime contractor is awarded the project and the pre-construction meeting is held, subcontractors may not be changed or substituted except under specific circumstances including failure to execute a contract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure or refusal to perform the subcontract, substantially unsatisfactory work performance, or substantial delays to project progress. Small Business Enterprise (SBE) or Minority Women Business Enterprise (MWBE) subcontractors used to meet SBE percentage requirements must be replaced with SBE subcontractors if removed, unless the prime contractor demonstrates SBE requirements are met through alternative means. Requests for subcontractor changes must be submitted in writing to the Procurement & Supply Management Contracts Compliance Manager. The worksheet must be completed and submitted with the bid as a mandatory requirement.
