About this file

This is a Request for Quotation (RFQ) from the Ohio Department of Natural Resources (DNR) for repairs to floor drains at Stonelick State Park located in Pleasant Plain, Ohio. The project involves replacing floor drains and traps in 10 bathrooms throughout the park, necessitated by previous floor refinishing work in 2024 that resulted in epoxy clogging the existing drain systems. The contractor will be required to cut and demolish concrete, remove and replace floor drains and traps, inspect pipe conditions, and restore the floor to its previous height while ensuring proper drainage. Work is to be completed during normal business hours (Monday-Friday, 8am-5pm) with a project completion/delivery date of March 1, 2025. Potential bidders are encouraged to schedule a site visit with DeWayne Terwilleger, the NRT Supervisor/Maintenance Manager, prior to submitting a bid.

The contract will be awarded to the lowest responsive and responsible bidder, determined by multiplying unit costs by estimated usage. Bidders must provide two examples of similar completed projects with photos, contact information, and participate in a pre-award meeting to ensure mutual understanding of materials and scope. Invoicing will be processed through Ohio Shared Services (OSS), with specific requirements for invoice submission, including a purchase order number, detailed service description, and compliance with state guidelines. The purchase order will be valid through June 30, 2025, with any services after that date requiring a new purchase order. The project does not include state sales tax, and the total contract value is not specified in the document.
