STATEMENT OF WORK
A. GENERAL INFORMATION
1. Title of Project: Automatic Door Opener/Closer
2. Scope of Work: The contractor shall provide all resources necessary to furnish and install automatic, surface mount door closures per floor map and Statement of Work on the 11th and 12th floor at the Region IX Office located at 1111 Broadway, Suite 1100, Oakland, California, 94607.
3. Performance Period: The contractor shall complete all work required under this SOW within four (4) months after scheduling with the FEMA Point of Contact, unless otherwise directed by the Contracting Officer (CO).
All scheduling of services shall be mutually agreed upon between the Government and the Contactor prior to the start of services. The contractor shall be given 24-hour notice for when work is required and requested.
Work performed is expected to be during and outside of regular business hours, Monday through Friday, 8am to 4pm. Any loud-work (drilling, demolition) performed outside the regular business hours shall be approved by the CO and coordinated with the on-site point of contact.
B. GENERAL
Work shall proceed expeditiously and during schedule timeframes as coordinated with the on-site point of contact. After-hours work may be approved for this project to limit the impact to the Government.
Contractor will provide their employees with necessary safety equipment.
C. STATEMENT OF WORK
Furnish and install automatic door closures
1. Number of doors to be outfitted with closures: 11th floor (10 units); 12th floor (8 units)
2. Type of installation – surface mount atop door frame
3. Replace door frames to accept new closure assembly
4. Sync door closures to actuator boxes and wall-mounted push-pads
5. Sync adjacent card readers to respond to push-pad activation
6. Powder coat and paint restroom closures to match paint color specified by FEMA project manager
6. Dispose of all waste materials in offsite landfill or recycling
G. PERMITS, ROYALTIES, AND LICENSES
The contractor shall obtain and pay the costs of any royalties and licenses for any patented or copyrighted items used in the performance of work.
Insurance requirements for successful vendor (proof required and submitted before work to begin).
a. Workers compensation Insurance as required by state law.
b. Commercial general liability insurance in the minimum amount of three million dollars.
c. Employer’s liability insurance in the minimum amount of three million dollars.
d. Business automobile insurance in the minimum amount of three million dollars.
H. Safety
The contractor shall perform all its activities pursuant to this contract in a safe manner.
The contractor shall assume responsibility on the job site for the actions of all its personnel and subcontractor(s) who are associated with performance on this contract. The contractor shall take adequate measures to prevent injury to the public or Authority property on the job site.
